This job was posted by https://idahoworks.gov : For more information, please see: https://idahoworks.gov/jobs/2339025 Blue Cross of Idaho is seeking a Transition Coordinator to support the goals and objectives of Blue Cross of Idaho\'s Transition of Care initiatives. These services which may include reaching out to members and providers to coordinate, implement or support already coordinated services initiated by clinicians through the transition from one setting to another.
This is an onsite role located at our Meridian, Idaho campus and will offer a hybrid work schedule.
We\'re looking for a Coordinator with:
Experience: 3 years\' relevant experience
Licenses/Certifications: Must hold one of the following:
- Valid unrestricted Idaho Licensed Practical Nurse (LPN)
- Valid unrestricted Medical Assistant Certification
What a day of a Coordinator would look like:
- Receives, and evaluates information received through the electronic medical management system and recommends services to clinician.
- Review\'s member eligibility, Health Insurance Portability & Accountability Act (HIPAA) warnings, and benefit issues prior to contacting or speaking with the member.
- Contact\'s members and providers to support the coordination of discharge planning services from either an inpatient or outpatient setting.
- Provides condition specific status to members and providers, as appropriate.
- Identifies members for on-going chronic care needs and refers to appropriate program/services.
- Documents and reports potential problems regarding quality of care or service issues through established protocols.
- Conducts member education and advocacy, as needed.
- Acts as a resource to others.
- Performs other duties and responsibilities as assigned.

As of the date of this posting, a good faith estimate of the current pay range is \$23.11 to \$ 32.36. The position is eligible for an annual incentive bonus (variable depending on company and employee performance.
The pay range for this position takes into account a wide range of factors including, but not limited to, specific competencies, relevant education, qualifications, certifications, relevant experience, skills, seniority, performance, shift, travel requirements, internal equity, geography, business or organizational needs, and alignment with market data. At Blue Cross of Idaho, it is not typical for an individual to be hired at or near the top range for the position. Compensation decisions are dependent on factors and circumstances at the time of offer.
