At St. Luke’s, our dedicated team of Nurse Navigators are critical in providing exceptional, patient-centered care. We are currently hiring for a Nurse Navigator at the SL Nephrology Clinic!

What You Can Expect:

The Nurse Navigator will work with providers to ensure the best care of the patients as they go through the stages of kidney disease. The team will work to preserve kidney function and prolong transitioning to dialysis, identifying, and referring potential kidney transplant recipients as timely as possible, and educating patients on available dialysis modalities.

Monday - Friday, 8am - 5pm. No Weekends / No Holidays.

Qualifications:

• Education: Bachelors of Nursing Degree

• Experience: 2 years's of RN experience

• Licenses/Certifications: Current RN License in the State of Practice

Preferred Qualifications:

• Previous Dialysis experience
