Job Summary Assists Case Managers with administrative duties and transportation for clients. Assists in the coordination of an integrated delivery system of care for all clients. * * Key Responsibilities 1. Provides relevant feedback regarding client to Case Manager, which assists the Case Manager in monitoring the results of the ISP to determine the need to reassess client or revise ISP accordingly. 2. Assists Case Managers in providing supportive interventions to clients including direct teaching interventions to assist clients in the development and/or practice and maintenance of needed adult-living skills. Participates in maintaining a clean working environment for staff and clients. 3. Transports clients to medical appointments and other appointments deemed necessary by Case Manager. 4. Assists in all required duties regarding the maintenance of all program-related and administrative data/reports, statistical records and other data collection activities as specified by Rushford. 5. Provides immediate crisis intervention, as needed, referring clients to Crisis Program, Case Manager, or Clinician as indicated. 6. Performs other duties as may be required. * Associate's degree in a mental health or human service discipline or applicable life experience. * Requires current, valid Connecticut driver's license. Must qualify for Public Service License. * Previous work experience with individuals with severe and persistent mental illness We take great care of careers.
