Position Summary The Health Related Social Needs (HRSN) Coordinator assists the Health Related Social Needs (HRSN) Manager in the implementation of the Community Capacity Building Funding (CCBF) to build capacity to provide Health Related Social Needs (HRSN) services across our programs and 15 county service area. This includes developing a deep understanding of CCBF and HRSN as well as assisting with conducting on going analysis of implementation strategies within our programs and processes. The Health Related Social Needs (HRSN) Coordinator will assist with developing program processes, providing training on new processes to internal stakeholders, and collaboration with funders. This position will act as a liaison between agency staff and the HRSN payors. This position will evaluate implementation and identify areas for improvement. This position will provide meeting facilitation, coverage in the manager\'s absence and identification of data collection and monitoring improvement opportunities and help coordinate communication across the programs. Essential Functions and Responsibilities Work with the Health Related Social Needs (HRSN) Manager to support comprehensive implementation of HRSN services for clients Ensure that staff understand and follow the HRSN processes Identify areas needing clarity of rules, guidelines, policies and program guidance for direct service staff Understand program requirements and clearly explain information to staff Manage program databases, spreadsheets and other data collection tools Train and support to staff on data and compliance Respond to staff data needs and inquiries and ensure their concerns are addressed Work with the Health Related Social Needs (HRSN) Manager to prepare timely quarterly reports for the Board of Directors and progress/final reports for program contracts/grants Create tools to communicate data findings to staff, clients, and community partners Participate in team meetings Work with the Health Related Social Needs (HRSN) Manager to develop quality assurance and improvement plans and assist with implementation Ensure compliance with HIPAA and all related confidentiality/privacy legal requirements Other duties as assigned For a complete job description, please visit: https://hivalliance.criterionhcm.com/jobs/51/1/#587
