Req# 29543 Aultman Health Foundation, Massillon, OH AULTMAN CENTER Aultman Health Foundation Full Time, Day Shift, 0630-1630 Job Summary: Functions in an expanded role as an advanced practice professional, performing acts of medical diagnosis and prescription of medical therapeutics or procedures, in collaboration with a physician, performing duties in both inpatient (as required) and outpatient settings. The APP works with an interdisciplinary team of primary caregivers and administrative support staff to perform assigned duties while displaying the highest standards of ethical and moral conduct as well as acting in the best interest of the Aultman Hospital, supporting our mission, vision, and values to optimize the health and well-being of our patients. PURPOSE OF POSITION The APP works in collaboration with the physician(s) to assess the physical/psychosocial needs of the chronic pain management patient. This APP can provide care for these patients in outpatient arena - Aultman Center for Pain Management. RESPONSIBILITIES & EXPECTATIONS Performs initial and ongoing assessment of patient's medical, physical, and psychosocial status, including conducts complete history and physical assessments, then develops and implements treatment plans. Performs rounds, records progress notes, orders tests, examinations, medications and therapies, provides consultations, and writes discharge summaries, based on interpretation of data. Provides medical direction to clinical staff. Performs specialized procedures as trained. Possesses excellent verbal and written communication skills. Employment status is contingent on the candidate's success in credentialing as a member of our Allied Health Staff. Maintains professional development within the APRN role. PARTICIPATION IN HOSPITAL PROGRAM ACTIVITIES (AS MUTUALLY AGREEABLE): Aultman Hospital Committees (as requested) Integrative Health Collaborative Membership and Committees (as requested) OTHER JOB RESPONSIBILITIES: Promotes health education in conjunction with other health professionals Efficiently uses group practice and healthcare resources Contributes to quality improvement activities by actively participating in medical staff activities, serving on various committees, including Integrated Health Care Collaborative initiatives, and suggesting and supporting process improvements EDUCATION/CERTIFICATION/LICENSING REQUIREMENTS: Graduate of an accredited Master of Nursing program. Certification by ANCC Credentialed by the medical staff in accordance with current policy and procedure (if applicable). Maintain BLS/ACLS (if applicable) provider certification and provide evidence of these. Five (5) years of clinical experience A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program All privileges are conducted in accordance with the established Standard Care Arrangement (SCA), within the scope of practice for the APRN, and in collaboration with the physician(s) identified by the SCA.
