Clinical Assistant

Duties and Responsibilities

• Triages patient calls and facilitates patient inquiries and requests.

• Tracking patients’ hospitalization status by communicating with facilities and social work teams.

• Communicating professionally and efficiently with physicians to help the clinical team obtain and clarify orders

• Facilitates communication between contract vendors and clinical teams and obtains clinical documentation timely

• Facilitates the ordering and delivery of supplies to patients' homes.

• Completes the required documentation to ensure that all activities are appropriately noted.

• Participates in data collection for patient surveys.

• Assist the clinical management team in follow-up with nurses regarding documentation and compliance issues.

• Performs all related duties to enable the efficient operations of the agency.

• Performs all clerical duties necessary to support the efforts of the clinical teams.

• Completes work, and documentation with accuracy and within agency time frames.

• Requires minimal supervision and is self-directed.

• Understands the delivery of service within home care.

  Education, Knowledge, Skills and Abilities Preferred

• Excellent customer service skills.

• High School Diploma.

• Experience in health care or a related field.

• Effective verbal and written communication.

• Sound judgment and critical thinking abilities.

• Excellent organizational and interpersonal skills.

• Computer literacy.

• Bilingual a plus.
