Responsibilities The Partial Hospitalization Program (PHP) Case Manager provides services within the PHP service line for individuals with psychiatric needs. Services may be offered in person or remote based on patient need, though the Case Manager is based out of our locations, in person. The Case Manager focuses on facilitating admissions, psychoeducation groups, treatment planning, discharge planning, care coordination, communication with the patient's support system, community providers, and others involved in the care of patients. Case Managers collaborate daily with psychiatric medical providers and therapists on the patient's primary Treatment Team to advance the plan of care within the average patient length of stay. Case Managers are supported by team leads, interdisciplinary team members, supervisors and directors. Accountable to uphold and apply knowledge and understanding of emotional and behavioral health practices and principles Responsible to assess, plan, implement, and evaluate the services for patients and share expertise with members of the Treatment team Develop and revise treatment and discharge plans and communicate these with patients, their support system, and the treatment team Responsible for communicating with patients and their support system to keep them involved in treatment plan decisions Provide psychoeducation to patients, their support system, and the treatment team, regarding admission process, diagnosis, symptom management, recommendations, discharge plans, safety plans, patient rights, financial impact issues, and other pertinent patient care matters Responsible for leading psychoeducation groups within the program on a daily basis Be oriented to ethically provide and advocate for patient-centered, culturally appropriate, age-appropriate care Maintains current competency in effective de-escalation of patients using Collaborative Problem Solving, therapeutic self, relationship-based care, and trauma informed care, as applicable to role. Qualifications Bachelor's in Psychology, Sociology, Human Services, or a closely related field required. Master's Degree in Counseling, Marriage and Family Therapy, Psychology, Human Services, or other Social Service related concentration preferred. Experience in healthcare setting preferred Experience using electronic health records preferred Knowledge of Microsoft Office Products including Word and Outlook and Teams Able to effectively and efficiently communicate in written and verbal form Ability to smoothly and rapidly shift between dissimilar tasks PrairieCare is a psychiatric healthcare system experiencing rapid growth in the Twin Cities and southern Minnesota.
