Community Living Counselor - Milner House Group Home

Location:Plainfield, CT

Seeking Community Living Counselors

Looking for dedicated, hard-working professionals to work in our licensed Transitional Living Program in Moosup, Connecticut. Working with our adults in mental health recovery to return to living independently. Work as part of a team providing individual support and group counseling in a residential setting. Milner House is a 14-bed intensively staffed transitional living facility. It serves people with severe and prolonged mental illness. The facility provides 24-hour coverage on three awake-shifts and is licensed by the Connecticut Department of Public Health. Work includes assisting and teaching clients in adult daily living skills and social and emotional skill development. A valid drivers license, reliable transportation and ability to transport clients in own car is required.

Responsibilities include:

  • Medication monitoring
  • Client transportation
  • Assists clients in addressing medical and psychiatric needs
  • Prepares and facilitates educational and skill building groups and one to one interventions as directed by the treatment plan
  • Assist in the preparation of meals
  • Individual and group counseling
  • Crisis intervention and risk assessment
  • Function as part of a team in support of client services
  • Coordinate care with providers and families as needed
  • Documentation
  • House cleaning

Comprehensive training provided for all positions.

Schedule:

  • 2nd shift - Full-time (Friday - Tuesday 2:45 p.m. - 11:15 p.m.) May include flexing and some holidays. Requirements: Associates Degree plus two years of appropriate experience required or Bachelors Degree in relevant human services field.
  • 2nd shift - Full-time (Tuesday - Saturday 2:45 p.m. - 11:15 p.m.) May include flexing and some holidays. Requirements: Associates Degree plus two years of appropriate experience required or Bachelors Degree in relevant human services field.
  • 3rd shift - (32 Hours /Part-Time) Friday - Monday 11:00 p.m. - 7:30 a.m. Requirements High School Diploma plus four years of relevant experience required. May include flexing and some holidays. (Benefit eligible).

Community Living Counselor Requirements:

1st and 2nd shift position - Associates Degree plus two years of appropriate experience required or Bachelors Degree in relevant human services field.

3rd shift position - High School Diploma or equivalent plus four years of appropriate experience required.

United Services, Inc. requires all employees to be vaccinated against influenza annually.

Community Living Counselor Salary Range:$18.54 - $20.98 per hour depending on experience

Bilingual Spanish/English Fluency:$19.48 - $22.04 per hour depending on experience.
