Case Managers:

  • Community Support Program

Located in Mansfield Center, CT and Dayville, Connecticut

(Position is located 30 minutes from Providence, RI - Worcester, MA - New London, CT - Willimantic, CT)

Case Manager Description: As a case manager you have a unique opportunity to forge a therapeutic connection with an adult who has been diagnosed with a mental health disorder. When individuals are asked what made the difference in their lives, the most frequent response is that they had someone they worked with who gave them hope that they can have what everyone wants: safe and decent housing, access to quality mental health and medical care, meaningful personal relationships, productive activities including work, and a connection to the community in which they live.

This is a job in which every day is different, where you have the independence and flexibility to structure your time but the support and back up of seasoned and professional supervisors as well as the opportunity to collaborate with members of our clinical and community support teams.

This position involves extensive work in community. Use of agency vehicles as well as use of own car (mileage reimbursement).

CASE MANAGER CURRENT OPENINGS:

Community Support Programis a multidisciplinary team providing case management, housing services, individual support and group counseling. If you have experience with mental health, substance abuse, community support services and an understanding of the psychosocial issues of this population, please consider becoming part of our team.

Case Manager Schedule (CSP): Flexible schedule Monday through Friday 8:30 a.m. to 5:00 p.m. with one evening until 7pm per week.

Young Adult Services Program (YAS)serves young adults, ages 18 to 25, who live in the Willimantic area and have a diagnoses mental illness. YAS provides not only clinical treatment but also continuity of care with community-based approach including case management

Case Manager Schedule (YAS): Monday - Thursday 9:00 am - 5:30 pm and Friday 8:30 am - 5:00 pm.

Case Manager Minimum Requirements:Associates Degree in relevant human services field plus two years of relevant experience or Bachelors Degree in relevant human services field.

United Services, Inc. requires all employees to be vaccinated against influenza annually during flu season.

Case Manager Salary Range:$18.54 - $20.98 per hour depending on experience

Bilingual Spanish/English Fluency:$19.48 - $22.04 per hour depending on experience
