Fedcap Community Navigator in Manchester, New Hampshire

JOB SUMMARY

Community Navigators are staff who may have personal experience with child protection services and who understand how the NH child welfare system works. Community Navigators have demonstrated their own ability to overcome adversity, achieve positive outcomes and support others. Community Navigators with no personal child welfare experience must have experience, lived, or worked, with supporting families involved within child welfare. The successful applicant will also have effective communication skills to educate professional reporters and families. The successful applicant requires solid knowledge of regional community resources for referring reporters and families to services. Community Navigators are trained peer professionals that can offer a wealth of knowledge and experience to reporters and families.

ESSENTIAL FUNCTIONS

  • Complete training in: Mandated Reporting, Motivational Interviewing, Diversity, Equity, and Inclusion (DEI), Trauma Informed Practice, Reflective Practice, Strategic Sharing, DCYF 101 and DCYF CORE Academy trainings.

  • Educate professional and nonprofessional reporters on local resources for families in need and who do not rise to the level of involvement with child protection.

  • Educate professional reporters on engagement techniques including trauma informed, reflective listening and motivational interviewing with families.

  • Educate professional reporters on facilitated referrals.

  • Outreach to families using specific legal language provided by the division.

  • Follow up outreach on referrals and warm hand offs.

  • Data entry and management of outcomes with reporters and families

  • Continued research and education on regional resources including Family Resource Center program schedules.

  • Partnership and collaboration with local resources, having firsthand

  • knowledge of what families can expect when they engage in services

  • Research, update, and manage resources throughout New Hampshire using database tools.

  • Complete annual training in engagement skills, engagement techniques, DEI, Trauma Informed and Motivational Interviewing.

  • Ability to pivot and work in ambiguity, understanding community resources change and family’s needs will vary.

  • Administrative work including case documentation, written correspondence and other data entry duties as needed.

  • Staff Meeting and Supervision attendance as scheduled.

  • Practice cultural competence.

  • Always display and maintain confidentiality.

QUALIFICATIONS

Required Education and Job Experience

  • High School Diploma, GED or HiSET.

  • Experience with DCYF/Child protection services.

  • Experience as a resource navigator.

  • Verbal and written communication.

  • Computer skills including data entry and data management.

Knowledge, Skills, and Abilities

  • MS Office intermediate skills in Word, Excel, and all Office 365 products.

  • Build strong interpersonal relationships with internal and external stakeholders.

  • Work cooperatively with others on a team.

  • Excellent verbal/written communication skills.

  • Excellent organizational skills and attention to detail.

  • Maintain the highest level of confidentiality, integrity, and ethical practice.

  • Knowledge of family court process.

  • Experience with Child Welfare system (lived or professional).

  • Analyze, make decisions, and resolve situations/problems effectively within the scope of responsibilities.

  • Demonstrable knowledge of:

  • Mission, vision and goals of Granite Pathways and The Fedcap Group and execution of relevant policies and procedures.

  • Strong organizational, administrative, and communication skills (both verbal and written).

  • Technical skills including Microsoft office products, Adobe, Oracle, and case management systems.

  • An understanding of statewide resources.

  • Demonstrable ability to:

  • Handle sensitive information in a confidential manner.

  • Work independently with minimum supervision.

  • Use analytical and critical thinking skills in supporting professional callers and families.

  • Maintain professional appearance and boundaries.

Physical Demands

  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or listen. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Work Conditions

Work is typically performed in an office environment but may be required to perform job duties outside of the office setting.

Position Type/Expected Hours of Work

Normal business hours: At times, must be willing to work beyond normal business hours as needed by operational needs and event schedules.

Travel

Travel will be required as needed to support the objectives of Granite Pathways contracted programs.
