JOB REQUIREMENTS: Social Worker - Sickle Cell and Hematology - 31633 DESCRIPTION/RESPONSIBILITIES: Work Schedule : Full-time, 100% FTE, day shift. Shifts are scheduled Monday - Friday between the hours of 8;00am - 5:00pm, no weekends required. Hours may vary based on the operational needs of the department. We are seeking a Social Worker to: * Work collaboratively as part of a multidisciplinary team to support patient engagement and positive health outcomes. * Evaluate complex patient and family situations which have legal, ethical, and psychosocial components. * Network proactively with and demonstrate a working knowledge of community resources. * Engage patient/family in the treatment and decision-making process with respect to patient rights, principles of confidentiality, respect for patient privacy, and right to self-determination.
