JOB REQUIREMENTS: RN Home Based Hospital Care - 33419 DESCRIPTION/RESPONSIBILITIES: Description: Work Schedule : 80% FTE, day/evening shift. On-call responsibilities. Weekend and holiday rotation. Hours are based off the operational needs of the department. You will travel and work primarily in patient\'s homes, your assigned work location will be 301 S. Westfield Rd., Madison, WI. We are seeking a Registered Nurse (RN): * Work directly with patients and families in the comfort of their home utilizing care plans, interventions and equipment that are modeled after an acute care inpatient unit setting. * Perform comprehensive assessments of physical, functional, psychosocial, and cognitive status of the Home-Based Hospital Care patient utilizing interview observations and physical exam techniques. * Assess post-acute needs and ensure that appropriate referrals to other services are made. Interdisciplinary conferencing is a key component of this role\'s responsibility, and thorough and timely documentation in EPIC is required.
