Position Description

The Breast Health Nurse navigator, is under the direction of the surgery clinic Manager and Physician Director of Breast Health and collaboratively assesses, plans, and coordinates timely delivery of high-quality, cost-effective care for women with breast cancer and breast health issues, including procedures and related care. The Nurse Navigator serves as the patient’s primary point of contact, ensuring seamless transitions throughout the continuum of care.

The Nurse Navigator works to optimize clinical integration by improving coordination and reducing communication barriers and inefficiencies across multiple specialties throughout the care path of breast cancer, including Radiology, Surgery, Oncology, Radiation Oncology, Genetics, Plastic Surgery and Survivorship. As liaison between care team members and the patient, the Nurse Navigator helps to coordinate case materials for collaborative review and Breast Tumor Board presentations when necessary.

This is a full-time position located at the DSMOC Surgery Clinic, although some of the non-patient facing duties can performed off-site and or outside of normal business hours.

Qualifications\Required:

State licensure as a Registered Nurse in Wisconsin with 3-5 years of breast health nursing experience or experience in a related area. Certified breast patient navigator from nationally accredited program (existing certification or obtained within first year of employment) Direct patient education experience, with ability to recognize and be responsive to each patient’s level of understanding Proven ability to effectively lead collaborative multidisciplinary teams Pleasant and friendly interactive style Excellent communications skills, both with patients and all levels of staff and providers Be able to provide social and emotional support for breast cancer patients and their families Strong analytical, problem solving and decision-making skills Demonstrated organizational skills in simultaneously managing multiple projects

Ideal Candidate\Preferred:

Previous experience in telephone triage or similar function Experience in a breast health program, oncology or general surgery practice Ability to develop, implement and evaluate performance metrics

Responsibilities:

Provide a seamless transition/hand off from the Radiology Breast navigator once a diagnosis of breast cancer or high risk lesion is identified Work in conjunction with the surgery nurses to coordinate surgical care Acts as a patient/family advocate providing information to facilitate informed care decisions, communicating results, and assisting with direct scheduling for coordinated care. Knowledgeable in Breast cancer standards from the NCCN and ASBRS. Facilitates communication, both written and verbal, to accurately coordinate care between patient/family and all members of the healthcare team including community agencies. Acts as a community breast health advocate representing SSM Health at community events. Maintains clinical expertise by attending educational programs and workshops. Maintains membership in relevant professional organizations to ensure awareness and incorporation of current best practices, technological advances, and evolving care models. Consistently models exceptional customer service, introducing first-time referring providers and referred patients to the navigation process. Presents detailed information to providers on each case and assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Maintains relationships with referral network. Contributes to development of teaching protocols and toolkits for patient education, family counseling and general public information. Assists in the development and implementation of policies, protocols and quality assurance standards. Monitors and provides input regarding specific operational and clinical performance metrics Utilizes the nursing process: assessment, planning, intervention, and evaluation in all patient encounters. Provides complete documentation of all encounters in electronic medical record. Performs other duties as required.

PRIMARY RESPONSIBILITIES

  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.

  • Collaborates with physicians and advanced practice providers to ensure quality outcomes. Coordinates efforts for treatment and promotes quality improvement and educational efforts.

  • Compiles data, tracks outcomes, and makes recommendations for process improvement.

  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.

  • Utilizes the nursing process of assessment, planning, intervention, and evaluation in all patient encounters.

  • Assists medical provider with processing of diagnostic results bringing significant values to the provider’s attention. Exhibits knowledge of tests and procedures and their significance as related to patient care.

  • Recognizes the implementation of dosages, interactions, side effects, adverse effects, routes of administration of drugs as they apply to individual patients. Uses knowledge of medications in instructing patients about medication therapy.

  • Establishes working relationships teaching protocols for patient education, family counseling and general public information.

  • Works in a constant state of alertness and safe manner.

  • Performs other duties as assigned.

EDUCATION

  • Graduate of accredited school of nursing or education equivalency for licensing

EXPERIENCE

  • Three years' registered nurse experience

PHYSICAL REQUIREMENTS

  • Constant use of speech to share information through oral communication.

  • Constant standing and walking.

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.

  • Frequent sitting, reaching and keyboard use/data entry.

  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.

  • Frequent use of smell to detect/recognize odors.

  • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.

  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.

  • Occasional lifting/moving of patients.

  • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.

  • Occasional driving.

  • Rare crawling and running.
