PruittHealth Hospice Chaplain - PRN in Macon, Georgia

POSITION: HOSPICE CHAPLAIN - PRN ( at least 1 unit of CPE is mandatory)

JOB PURPOSE:

To plan and coordinate spiritual services for United Hospice families and patients.

KEY RESPONSIBILITIES:

  1. Ability to interact with a variety of different age groups.

  2. Ability to work well independently and within an interdisciplinary team setting.

  3. Ability to communicate effectively orally and in writing with individuals and groups.

  4. Ability to handle confidential matters.

  5. Ability to work a flexible schedule.

  6. Possess knowledge of computer software, including proficiency in word-processing.

  7. Provide spiritual services to patients and families based upon their identified need (spiritual values and beliefs are NOT to be imposed upon the patient/family).

  8. Assess the spiritual needs/concerns of patients and families through direct communication with the individual, the patient’s clergy, or by information obtained by other Hospice personnel.

  9. Ensure that spiritual support is available to patients and families in keeping with their wishes and desires.

  10. Provide consultation education and support for other members of the hospice team, healthcare centers and facilities, and the community.

  11. Performs sacerdotal functions as necessary to palliate the spiritual crises of patients and families.

  12. Assist with volunteer training as requested.

  13. Serve as a member of the Interdisciplinary Group to assist in identifying and addressing spiritual needs and developing care plans, including attendance at team conferences for residents of healthcare centers and nursing facilities.

  14. Assist with on-call chaplaincy coverage.

  15. Act as a liaison to community clergy.

  16. Provide grief and bereavement counseling to families.

  17. Conduct memorial/funeral services as requested.

  18. Perform other duties as assigned.

  19. Keep supervisor informed of daily activities and issues.

  20. Must get along with coworkers, maintain effective relationships with management staff, employees, and the general public.

  21. Follows all company policies and procedures, state and federal laws and regulations and charged with reporting violations to the appropriate supervisor.

  23. Maintains confidentiality of company information.

  24. Demonstrates honesty and integrity at all times.

  25. Responds to change productively and handle additional projects as assigned.

  26. Able to carry out the duties and responsibilities of this job (with or without reasonable accommodation) without posing specific or current risk of substantial harm to the health and safety of self or others.

  27. Will coordinate patient care responsibilities with Director of Health Services and other members of the interdisciplinary team.

  28. Perform other duties as assigned by Administrator.

KNOWLEDGE, SKILLS, ABILITIES

• Ability to work effectively independently and within an interdisciplinary team, and across denominational/faith lines.

• Orientation to United Hospice and any other training that may be necessary to complete the assigned duties effectively.

• Satisfactory results of criminal history check and Office of the Inspector General Sanction Check

• Free from the illegal use of drugs, and free from the use and effect of drugs and alcohol in the workplace.

• Ability to work a flexible schedule.

• Excellent Organizational skills.

• Has never been shown by credible evidence (e.g., a court or jury, a department investigation, or other reliable evidence) to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected to any person to serious injury as a result of intentional or grossly negligent misconduct as evidenced by an oral or written statement to this effect obtained at the time of application.

• Orientation to United Hospice and any other training that may be necessary to complete the assigned duties effectively.

• Participate in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.

• Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.

• Attend and participate in mandatory inservices.

• Honor patients/residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.

• Comply with corporate compliance program.

• Report job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.

• Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.

• Follow established safety procedures when performing tasks and/or working with equipment.

• Perform other related duties as necessary and as directed by supervisor.

MINIMUM EDUCATION REQUIRED:

• Completion of a Master’s degree from an accredited theological school/seminary.

• Completion of 1-4 units in Clinical Pastoral Education from a nationally recognized provider.

• Equivalency or exceptions the above qualifications will be approved by the Director of Spiritual Care.

MINIMUM EXPERIENCE REQUIRED:

• Two years of ministerial experience.

• Appropriate endorsement within his/her denomination (e.g., ordination).

• Previous training and/or experience in death and loss issues.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

• Current Driver’s License and Automobile Liability Insurance
