UCLA Health Medical Home Visit Program Coordinator in Los Angeles, California

Description

Join our dedicated team as a Program Coordinator for the Medical Home Visit Program.

In this vital role, you will:

  • Support Providers and Leadership: Report directly to the program director and manager, providing essential support to healthcare providers and ensuring smooth program operations.

  • Patient and Physician Liaison: Act as a key liaison between patients and physicians, facilitating effective communication with referring doctors.

  • Quality Improvement Collaboration: Work on quality improvement projects to enhance program effectiveness and patient care.

  • Program Development: Assist in the development and implementation of program initiatives, collaborating closely with the program director, manager, and division administrator.

  • Administrative Support: Help plan and execute educational and training programs, schedule meetings, collect program materials, and manage logistics.

  • Stakeholder Communication: Liaise with university/hospital administration, community partners, patients, and volunteers to meet program needs and assist in preparing reports for stakeholders.

  • Task Prioritization and Reporting: Prioritize tasks to meet program objectives and provide status updates during weekly meetings.

Salary: $28.23- $40.44 hourly

Qualifications

Required:

  • Ability to establish and maintain aproductive working relationships with Faculty, Staff, Administration and patients from various social, cultural and economic backgrounds.

  • Ability to creatively integrate competing demands into a productive working environment.

  • Skill in verbal communications to clearly convey complex problems in both formal and informal situations.

  • Emotional intelligence to be able to understand student and patients' need and coordinate to fulfill that need, including the ability to interact diplomatically and sympathetically with a patient population in varying degrees of health.

  • Demonstrated skill in assisting with research, planning, development, and implementation of faculty and health professional programs.

  • Ability to organize and manipulate a large quantity of data using excel.

  • Demonstrate skill to coordinate meetings with Leadership and other Program Stakeholders using Microsoft Office Teams, Outlook or Zoom.

  • Skill in coordinating and prioritizing the necessary steps for program implementation by working with the program representative, faculty, and others to determine program needs and establish time, place, and sequence of steps to be taken.

  • Ability to work independently in prioritizing duties, problems, and formulate effective solutions with the program representative.

  • Working knowledge of UCLA policies and procedures preferred, including working on projects with external funding sources, and working knowledge of Institutional Review Board.

  • Ability to work occasional evenings and weekends

  • Accuracy and attention to detail

Preferred:

  • Working knowledge of patient related policies and procedures as well as a thorough understanding of the University system and it services and programs. Knowledge of hospital computer system, and personal computer systems and the various software products to perform administrative work.

  • Familiarity with Medicare and CMS programs
