UCLA Health Case Manager in Los Angeles, California

Description

Under the direction of the Program Director and Case Manager Supervisor at UCLA Operation Mend, you will provide comprehensive case management for Operation Mend veterans and family members. The Family Care Manager serves as a care manager for veterans and family members seeking care at UCLA Health Operation Mend for specialty physical and psychological injuries sustained in the line of post 9/11 military service.

You will work collaboratively with the Operation Mend multidisciplinary team to engage, educate, communicate, and coordinate care for assigned Operation Mend participants; maintain contact with relevant providers to monitor and coordinate treatment efforts and provide comprehensive treatment plan; maintain contact with relevant providers to monitor and coordinate treatment efforts and provide comprehensive treatment plan; works telephonically and/or using telehealth platform, for both behavioral and physical health issues to provide treatment plan, coordinate care with participants to maintain continuity of care, identify immediate needs, review treatment goals and implement action steps towards achieving goals; provides support in linking Operation Mend participants to their local veteran service organizations and other community support services; and provides follow-up check-ins via telehealth at 3-months, 6-months and 12-months to reassess the needs and goal attainment of all Operation Mend participants.

Salary Range: $64,800 - $137,600/annuallyQualifications

  • License eligible (LCSW, , LMFT, MSW, MFT)

  • Masters in psychology or related field.

  • Previous experience working with trauma, veterans, caregivers is a plus.

  • Experience working with families from diverse backgrounds.

  • Skilled in keeping accurate and timely records.

  • Excellent written and verbal communication skills.

  • Ability to work effectively and closely with other members of the team.

  • Excellent organizational and problem-solving skills.

  • Ability to work independently and report to Case Manager Supervisor and Operation Mend leadership on a regular basis and in a timely manner.

  • Skilled in management of time and setting priorities.

  • Ability to take direction.

  • Proficient in Microsoft Word, Outlook, Excel, and Access. Ability to work flexible hours. Ability to follow University Policy and Procedures.

  • Bilingual, a plus.
