Responsibilities

Under the supervision of the Program Manager the The HIV Health Navigation Specialist (HNS) is responsible for planning coordinating implementing and evaluating patient retention activities to re-engage patients living with HIV/AIDS back into medical care. The Health Navigation Specialist is a member of the Medical Care Coordination program team tasked with contacting patients who may be out of healthcare completely or underutilize services and provide brief structured interventions to engage patients back into will be a key role to ensuring that key populations who are disproportionately out of healthcare completely or underutilize available services are connected to culturally-competent and compassionate and comprehensive HIV services.

Essential Functions and Responsibilities:

  • Works with MCC team to identify locate and reengage patients back into HIV care.

  • Schedules follow-up appointments and conducts follow-up activities to ensure patients return for follow-up visits.

  • Assists in the coordination and delivery of patient-centered activities that focus on addressing health status engagement and retention in care adherence to HIV medications and HIV risk reduction.

  • Works independently conducting office and field investigations (site visits to patient’s homes neighborhoods shelters etc) to locate assess and refer individuals for necessary medical evaluation and treatment.

  • Tracks engagement and retention in medical care.

  • Maintains confidentiality of protected health information in compliance with state and federal law.

  • Participates in staff meetings Medical Care Coordination meetings case conferencing and other meetings as required.

  • Enters and maintains program data including clinical documentation in Electronic Health Records (EHR).

  • Enters monthly serviced data in Casewatch in a timely manner and prepare other demographic or statistical reports as required by the funding agency and Program Manager.

  • Document all services provided according to standards set forth in County of Los Angeles- Division of HVI and STD Programs (DHSP) funding contracts

  • Other duties as assigned.

Qualifications

  • Two years’ experience conducting outreach to engage individuals in HIV prevention or care.

  • Three years working in the HIV field

  • Three years supporting LGBTQ Latino Black street-involved behavioral illness drug-using and/or sex worker populations

  • Knowledge of Medi-Cal Medicare Ryan White and private insurance eligibility and enrollment structures.

  • Knowledge of HIV risk behaviors substance abuse STIs and behavior change principles and strategies.

  • Familiarity with social service resources in the local community preferred.

  • High level of compassion and ability to communicate clearly with at-risk populations

  • Proven track record in a team-centered work environment focused on collaboration

  • Ability to deliver care in an ethical nonjudgmental culturally competent and nondiscriminatory manner

  • Highly motivated and driven passion to take ownership of their role within the team

Preferred Skills & Knowledge:

  • Knowledge of and experience in infectious disease care and support services outreach health promotion and/or similar or related fields preferred

  • High level of compassion and ability to communicate clearly with at-risk populations

  • Proven track record in a team-centered work environment focused on collaboration

  • Ability to deliver care in an ethical nonjudgmental culturally competent and nondiscriminatory manner

  • Highly motivated and driven passion to take ownership of their role within the team

Pay Range

$23.00 - $30.11 /hour
