SUMMARY:

Provides leadership and expertise in the nursing care of patients with wound, ostomy and continence issues considering age specific, developmental, cultural, and spiritual needs. Promotes the use of current knowledge, techniques and research through practice, teaching and consultation; and serves as facilitator for change in the nursing practice. Performs professional nursing functions for selected patients. Demonstrate unit/area designated competencies.

Provides advanced nursing care in accordance with WOCN Scope of Practice to patients or groups of patients across the health care continuum in collaboration with the health care team. Provides informal consultative services for select patients in the community.

  • Selects appropriate ostomy and wound care products based on evaluation on patients’ needs and capabilities.

  • Develops, reviews and evaluates interdisciplinary plan of care within area of clinical expertise for selected groups or individual patients that includes patient’s identified goals. Collaborates with nursing staff and other disciplines as needed.

  • As the clinical expert for wound, ostomy, and the prevention of skin injuries sets direction for nursing practice consistent with SJM Ann Arbor and national standards (NPUAP). Utilizes evidence based practice and/or research findings to guide and direct practice changes.

  • Leads organizational initiatives in the prevention of hospital acquired pressure or skin injuries and the treatment of present on admission pressure and/or skin injuries.

  • Utilize the nursing process at an expert level to assess, diagnose and treat the complex responses of individuals, families or communities to actual or potential health problems.

  • Functions as an educator by teaching clients, families, communities and other health care providers; and serves as a role model and preceptor for other nurses in a variety of settings. Shares expertise with all members of the health care team through consultation and collaboration.

  • Takes leadership role for quality improvement initiative, providing in depth clinical expertise and critical analysis.

  • Provides clinical expertise and participates in the evaluation and selection of specific wound, incontinence and skin care products including specialty beds, mattresses, and supplies.

  • Participates in the identification, development and evaluation of patient teaching programs and patient education materials considering age specific, maturational, cultural, and spiritual needs.

  • Collaborates with nursing leadership in making recommendations/decisions regarding clinical practice issues and effective utilization of resources.

  • Participates on, or chairs various departmental, health system and medical staff committees/task forces.

  • Contributes to the professional growth of colleagues through educational program development, mentorship and consultation.

  • Is responsible for own continued learning and professional growth.

  • Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.

REQUIREMENTS:

Education: Bachelor Degree in Nursing. Completion of appropriate accredited WOCN Program within one year. Master Degree in Nursing preferred.

Experience : Three to five years progressively more responsible related work experiences.

Certification : Certification in appropriate specialties such as Wound, Ostomy, Continence Foot and Nail preferred.

Licensure: Current Michigan license as a Registered Nurse.
