Trinity Health Nurse Assistant - MG1 Stepdown Unit - Pool Tier III, Evening Shift (St. Mary) in Langhorne, Pennsylvania

Description:

Trinity Health Mid-Atlantic, is looking for a Nurse Assistant to join our team!

Schedule: Pool Tier III/Per Diem, 12hr evening shifts 11a-11:30p

Tier III Requirements: 96-hour minimum commitment per 4-week schedule, 3 weekend shifts per 4-week schedule, 1 winter holiday (Thanksgiving, Christmas, New Year's)

The primary objective of this position is to improve quality. To provide direct, basic patient care activities under the guidance and direction of a Registered Nurse. To provide care and cleaning of specific equipment, stocking of supplies and equipment and the transportation of patients, supplies and equipment.

  • Performs direct and indirect patient care activities following RN instructions, physician orders, policies, procedures and regulations Assures patients are clean, well-groomed, neat, free of odor, dry and properly dressed Uses proper body mechanics during all activities and duties

  • Practices proper hand washing and principles of infection control. Obtain supplies from designated supply rooms to assure patient rooms are ready for patient admission and cleared when patient discharged/transferred.

  • Assist RN to observe skin condition on all patient admissions and transfers. Displays competence while performing oral hygiene and skin care. Displays competence while performing weight and heights utilizing standing chair and bed scale.

  • Demonstrates ability to feed patients, knowledge of dysphasia precautions and calculating and recording intake and output. Demonstrates competence while collecting specimens-urine routine, clean catch, Foley, stools for c-diff, etc.

  • Demonstrates competence in perineal care and foley care. Displays competence in the care of isolation patients utilizing disease-specific precautions.

  • Performs and records vital signs accurately. Maintains competence with glucose monitoring utilizing standard precautions.

  • Accurately uses the bed alarm and chair alarm system when being utilized for fall prevention. Participates in hourly rounding programs and schedules to ensure patient safety and reduce patient harm; observes for environmental safety, informs patient and family on safety precautions, turns and repositions patients, preemptively assists with toileting activities, and addresses personal needs.

  • Transports patients safely keeps patient’s room neat and orderly reporting any maintenance or environmental service needs. Demonstrates competence for caring for patients under continuous observation related to fall risk, behavioral issues and suicide

  • Gives patient status report to registered nurse. Greets and directs visitors, patients, families and physicians appropriately Responds to Patients’ call lights promptly. Communicates any problems, concerns or issues promptly to the registered nurse .Follows verbal and written instructions accurately and in a timely manner

  • Works as a team member demonstrating a positive, supportive and helpful attitude towards patients, families, visitors, co-workers and physicians.

  • Acts as a preceptor for new employees as needed.

Position Requirements:

  • CNA Cert. or proof of successful completion of 1 semester of clinical rotation
