McLaren Health Care Coordinator Admissions - McLaren Careers in Lake Orion, Michigan

Position Summary:

As directed by the Executive Director, the Admissions/Discharge Coordinator manages patients through the continuum of care, promoting the delivery of quality, cost-effective health services based on medical necessity and contractual benefits. Emphasis on utilization review, documentation and transition planning. Works in coordination with care team members to develop programs and protocols to enhance the cost and quality of care delivery.

Primary Responsibilities:

  • Screens referrals within 4 hours of referral being made. Conducts a medical record review and interviews referral/responsible party to determine medical necessity, appropriate source of payment and overall appropriateness of admission within one working day following the initial screen. Confirms admission criteria guidelines. Provides clinical review to managed care agencies to obtain authorization/certification for services being provided, based on available insurance benefits. Approves/denies admission no later than 24 hours of initial referral/screen.

  • Responsible for obtaining a primary and secondary payer source prior to admission.

  • Responsible for obtaining a signed admission contract with patient/responsible party prior to admission.

  • Responsible to obtain any/all copies of insurance cards, advanced directives.

  • Identifies high-risk patients based on diagnosis, age and combination of medical and social features.

  • Perform other related duties as assigned and directed.

Qualifications:

  • High School Diploma/GED

  • Two years clerical experience in hospital, long term care, medical field setting

Preferred:

  • Admissions experience in Long Term care
