Hometown Waiver Solutions Direct Support Professional in Lafayette, Indiana

We are hiring for Part-Time Direct Support Professionals. Positions will work 10-25 hours per week, day and evening shifts available.

Purpose: To assist consumers in performing the functions of daily living with as much independence as possible. Under general supervision, to guide consumers to be self-directed by teaching consumers decision making skills and encouraging independence as defined in each person's Personal support/Habilitation Plan. To provide assistance in manner that supports the personal dignity of the consumers.

Essential Duties and Responsibilities

  • Shall comply with all rules, regulations, Policies and Procedures of the Company
  • Shall provide physical assistance to individuals as needed
  • Shall provide residential training to facilitate the acquisition of communication skills, sensorimotor skills, and daily living and self-help skills
  • Shall provide assistance with daily living, such as ambulation, dressing, grooming, eating, toileting, bathing, meal planning and preparation, laundry, budgeting and financial assistance, home care and cleaning
  • Shall provide emotional support, advice and assistance to help the individual work through problems
  • Shall communicate with the assigned Case Manager on a regularly scheduled basis and as needed
  • Shall provide or make arrangements for transportation to services, activities and medical appointments as needed
  • Shall actively participate in medical appointments and follow up care as directed by the medical staff
  • Shall cooperate with monitoring to include periodic announced and unannounced visits to the home by various monitoring entities
  • Shall assist individuals with the administration of their medications and ensure that medications are properly accounted for and properly labeled
  • Shall administer all aspects of residence operation; including development and maintenance of a household budget
  • Shall assist with menu planning and grocery shopping as needed
  • Shall participate in household cleaning if necessary
  • Provide direct support to consumers
  • Assist consumers in performing daily living functions such as personal hygiene, dining skills, household chores and community integration, etc.
  • Ensures all personal goals are worked on a daily basis and appropriate documentation is completed
  • Administers medications at the appropriate time (S); notifies management and completes Medication Error form if an error occurs
  • Assist with meal service; teach dining skills and promote social skills and personal dignity during meals
  • Attend and successfully complete in-service training sessions as scheduled
  • Ensures safe and clean living conditions at all times
  • Complete data collection, observation notes and daily reports as instructed
  • Ensures that individual has choice of community activities and provides safe transportation
  • Ensures/provides safe transport of consumer (in personal vehicle) to and from destination(s) as needed while on duty
  • Assess the development of the consumer's abilities and interests to provide information for their Personal Support/Habilitation Plan
  • Participate in Interdisciplinary Team meetings to formulate the consumer's Personal Support/Habilitation Plan
  • Monitor consumer's activities to assure that safety and health care practices are observed
  • Ensure Universal precaution practices are observed
  • Reports to the Field Coordinator all incidents
  • Maintains confidentiality at all times and ensure HIPPA laws are followed
  • Ensures all individuals retain cash receipt for all purchases; completes all appropriate financial documentation
  • Monitors, addresses per individual's plan documents each individual's behaviors
  • Documents incident or injury on appropriate forms; notifies supervisor immediately in event of the injury
  • Must maintain all compliance requirements outlined for the position of Direct Care Staff; Reliable i sured transportation, valid driver's licenses, current PPD (Mantoux TB test) and CPR and First Aid
  • Ensure/assists in follow-through regarding special diet, therapies, treatments and medications as prescribed
  • Perform related duties as required or assigned to fulfill the philosophy, goals and objective of the program site
  • Follows companies attendance policy and guidelines
  • Follows all policies and procedures as set fourth by Residential Living Services
  • Treat consumers with dignity and respect
  • Promote independence, choice and empowerment fo consumers
  • Provides quality care to each individual according to his/her needs
  • Reports to management any activities, which may jeopardize individuals health and safety will being or any occurrences that go beyond Residential Living Services rules of conduct
  • Shall plan and institute activities of choice for individuals living in the residence
  • Shall ensure that staff notes are legible and complete
  • Shall ensure that individual training objectives are documented to ensure the individual is meeting or working toward personal goals
  • Shall conduct randomly scheduled fire and evacuation drills in the home and document appropriately
  • Shall perform other duties as assigned by Field Coordinator or Operations Team

Qualifications:

  • Must have a High School Diploma or GED and be at least eighteen (18) years of age
  • Must be a US Citizen with a valid social security number or have appropriate work permit documentation
  • Must be proficient in the monitoring and completion of various reports and records
  • Must be proficient in handling crisis situations
  • Must pass a Criminal Record Check
