Position Summary:

The Nurse Navigator coordinates care from admission to discharge of Orthopedic and Spine patients. Leads the multidisciplinary Ortho/Spine team. Provides knowledge and skill in the care of Ortho and Spine patients and serves as a resource to staff and the interdisciplinary team and as a primary point of contact for patients and families in providing indirect patient care. Maintains quality assurance data with the goals of achieving clinical excellence, including any certification/accreditation requirements. Provides education and training in accordance to data results and trends identified. Assists in the development of policies and procedures, order sets, and pathways to improve quality of care and program development. Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to full recovery. Works with patients and families to answer questions, provide emotional support, and solve issues concerning the continuum of care.

Responsibilities

  • Serves as the essential link between patients and other care providers to provide resources and assistance with accessing clinical and supportive care services offered within the healthcare system and the community.

  • Reviews potential candidates for the Orthopedic Total Joint program. Screens patients preoperatively for the Ambulatory Joint program versus overnight hospital stay. Prepares list of joint patients that meet ambulatory criteria/surgeon orders and shares list with multidisciplinary team of care providers.

  • Assists patients in understanding their diagnosis and treatment options presented by their surgeon and anesthesia provider for Total Joint program clinical pathway.

  • Advocates on behalf of the patient.

  • Performs pre-operative and post-operative phone calls for Orthopedic Total Joint patients.

  • Visits Orthopedic Total Joint patients and families pre-operatively and post-operatively to ensure communication of care information and questions.

  • Teaches Orthopedic joint class; educates patients about their care pathway, pre and post nursing care process and surgical procedure.

  • Utilizes nursing skills, documentation, and surgical plan of care with patient and multidisciplinary team.

  • Collaborates and communicates patient concerns and/or potential changes to care plan with surgeon and multidisciplinary team.

  • Evaluates and monitors quality Total Joint metrics and reports findings at the quarterly Orthopedic service line meeting.

  • Educates and collaborates with nursing in the care of Total Joint patients.

  • Works to develop and maintain rapport with Orthopedic surgeons and all members of the patient support team.

  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.

  • Performs other duties as assigned.

Qualifications

Minimum Education:

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

Minimum Experience:

Minimum of three (3) years of Orthopedic nursing and/or surgical experience.

Licensure Requirements:

Must have and maintain state of Tennessee RN License.
