Overview
MHA in Ulster County, Inc. seeks a Full Time (37.5 hours a week) Assessment & Service Plan Specialist to work in our Assertive Community Treatment Team (ACT Program) providing intensive mental health treatment, rehabilitation, case management, and support services in a multi-disciplinary mobile team model, to 68 adults in Ulster County with a diagnosis of serious mental illness, and substance abuse disorders. This includes traveling throughout the community to see ACT participants in various community settings including their homes and educating recipients about self-help techniques, effective coping strategies, symptom management skills and assist in clarifying rehabilitation goals as well as developing a network of community supports. This position will have a primary focus of completing NYOMH required assessments, patient-centered service plans, and safety plans with all ACT participants. Services are constructed to meet individual needs, and are available on a 24 hour/7 day a week basis.
The schedule for this position is Monday-Friday 9am-5pm.
The pay rate for this position is $32.74 hourly.
Responsibilities and Duties Include
Work with the ACT Program Director to meet program/department goals and objectives, both independently and as a team, and ensure and maintain the confidentiality of all program participants and department information.
Completion of required assessments with ACT participants.
Develop and complete patient-centered service plans and safety plans with ACT participants.
Coordinate and integrate a written service plan with service providers in cooperation with clients.
Uses empirical supported techniques to address specific symptoms and behaviors.
Provide services that prevent or resolve crisis in order to avoid unnecessary use of emergency rooms and in-patient services.
Coordinate services with all significantly involved service providers and resources.
Identify service needs and/or gaps in services and advocate for system changes when indicated.
Provide 24 hour on-call services to clients on a staff rotation
Work 1-2 late evenings as needed by program.
Qualifications and Skills
Master's Degree in social work, psychology, mental health counseling or related human service degree and two years relevant experience required.
NYS Licensure preferred (LMSW, LMHC, CASAC).
Ability to safely perform the essential functions of the position, with or without accommodations required by federal, state, or local law.
Strong interpersonal skills and ability to work with people from diverse backgrounds and ability to communicate effectively, both orally and in writing.
A clean, valid NYS driver's license that will meet and maintain a satisfactory driving record consistent with agency and insurance standards.
