Purpose

Job Summary

The primary purpose of this position is to serve as Lifestyle Coach for the Wellness Department. This is an entry level position conducted by someone familiar with Tribal members and the local community, who will facilitate activities one-to-one or in a group setting, teach selected classes, or other approved curriculum, recruit participants, and work with the wellness team to coordinate and deliver effective services. This position will serve as a liaison for the Wellness department and its programs as directed.

Job Duties & Responsibilities

  • Implements program protocol in group and individual counseling sessions to assist participants in achieving individual health goals.

  • Instructs and assists participants in preventative health using concepts of healthy eating, stress reduction and physical activity.

  • Uses Motivational Interviewing counseling skills to assist participants with behavior, lifestyle modification to achieve individual health goals.

  • Participates in local team meetings and national study meetings and trainings required of Lifestyle Coach I.

  • Monitors participant safety and compliance with program protocol.

  • Assists team members with administrative, data collection, recruitment, retention and data entry as required to meet program requirements.

  • Participates in screening and evaluation activities: collect body measurements (height, weight, waist, skin folds, etc.) and administer questionnaires using interviewing skills, as directed by supervisor.

  • Prepares patient and participant correspondence, including telephone and written correspondence, as well as manages mailings and other outreach communications.

  • Collaborates with a multidisciplinary team

  • Utilizes population health registries to identify needs and to organize patient outreach, Verifies all data for accuracy.

  • Plans and participates in outreach activities to include Health Fairs, fitness campaigns and other community events.

  • Coordinates project activities as assigned and collaborates with other program and department staff.

  • Participates in KIC’s quality improvement processes by working closely with the Quality Team to coordinate improvement projects as applicable for population health initiatives.

  • Maintains confidentiality of all program information.

  • Flexible Scheduling. Hours of attendance may vary between 8:00 a.m. to 8:00 p.m. Monday – Saturday, to accommodate classes and education activities offered during the non-traditional work week.

  • Other duties as assigned

Minimum Qualifications (Education & Experience)

  • High School Diploma or GED required.

  • Two years or more work experience in an office setting.

  • Certification: Current BLS certification or able to obtain certification upon hire.

  • A combination of relevant experience, education, and training may substitute for experience requirements on a year-for-year basis.

  • Proof of valid driver's license with acceptable driving record.

Preferred Qualifications (Education & Experience)

  • Work in the health care field.

  • Experience in health promotion, wellness, nutrition and/or physical activity.

  • Clerical - Knowledge of administrative and clerical procedures and systems such as word processing, excel, PowerPoint managing files and records, designing forms, and other office procedures and terminology.

  • Customer and patient care service - Knowledge of principles and processes for providing customer and patient care services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.

  • Experience in community clinic preferred and a strong commitment to multi-disciplinary teamwork.

Necessary Knowledge, Skills, and Abilities

  • Strong understanding and sensitivity to American Indian/Alaska Native (AI/AN) Tribal culture and lifestyle, working effectively with AI/AN populations.

  • Effective oral and written communication skills with sensitivity to the lifestyles of targeted participants.

  • Skills in operating personal computers using a variety of computer software.

  • Ability to establish and maintain cooperative working relationships with management, coworkers, and outside contacts.

  • Ability to maintain strict confidentiality and comply with all applicable federal and tribal privacy laws and KIC policy (HIPAA, HITECH, etc.).

  • Ability to learn and apply program/agency operating policies and procedures.

  • Ability to manage multiple priorities and tasks concurrently and meet deadlines.

  • Ability to work independently as well as with teams.

Conditions of Hire

  • May be required to possess and maintain a valid driver's license or capable of obtaining one, as well as be insurable by KIC’s company insurance.

  • KIC is a Drug-Free Workplace. All employees must adhere to KIC drug and alcohol policies and procedures to ensure a safe workplace. Employees must pass a pre-employment and subsequent random and/or for cause drug and alcohol screening to be eligible for and maintain employment.

  • KIC has several positions that must comply with the P.L. 101-630 Indian Child Protection and Family Violence Protection Act (ICPA); all employment offers in the "covered" classification are conditional until KIC has received a Federal criminal background check verifying eligibility to work in these programs.
