The State of New Hampshire, Department of Health and Human Services, Division for Children, Youth, and Families has a full time vacancy for Case Support Specialist.

SUMMARY:

Reports to the assigned DCYF District Office Supervisor, preforms diverse supportive and administrative duties to assist the Division for Children, Youth and Families (DCYF) to achieve the daily objectives of the division within the district offices. To support timely case management procedures and permanency for every child / youth through collaboration with family and community partners, developing plans of action which promotes healthy interactions. Upholds the vision and mission of the agency through utilizing the Practice Model theories, beliefs and principles in daily practice.

RESPONSIBILITIES:

Explains relevant processes and conduct interviews with collateral witnesses by telephone and/or in person. Collateral contacts may include: custodial parents, non-custodial parents, physicians, teachers, and others who may have witnessed alleged child abuse or neglect.

Engage in assessment procedures to investigate alleged child abuse and neglect in overdue assessments and submits completed investigations for supervisory case closure review.

Maintain list of community providers and confidential assessment/case records consistent with DCYF protocols and responds to questions from consumers and providers.

Provides periodic administrative support services, as needed, under the instruction of the DCYF District Office Supervisor, this may include copying requests for discovery, social histories, record retention, case file management activities.

Transportation of children / youth and their families to activities such as visits, scheduled appointments, court.

Contacts consumers and provides supervision of Face to Face visits between children / youth and their parents custodial/ guardian.

Contacts recipients to arrange scheduling of medical appointments for follow-up diagnosis and treatment plan requirements.

Keeps records of appointments and updates case files and electronic management system .

MINIMUM QUALIFICATIONS:

Education: Possession of an Associate's degree or completion of two years at a recognized college or university. Each additional year of approved formal education may be substituted for one year of required work experience.

Experience: Two years' experience in the field of social welfare in a position such as child care worker, teacher's aide, therapist assistant in a rehabilitation program, or public health aide in a health agency. Each additional year of approved work experience may be substituted for one year of required formal education.

License/Certification:

Possession of a valid driver's license and/or have access to transportation for statewide travel.
