DEPARTMENT: Jamestown Treatment Apartment Program - Jamestown, NY SCHEDULE: Sunday, 9:00 am to 5:00 pm Monday, 9:00 am to 5:00 pm Tuesday, 9:00 am to 5:00 pm Wednesday, 9:00 am to 5:00 pm Thursday, 9:00 am to 5:00 pm
SALARY: Minimum $19.57/hr
JOB FUNCTION:
Provide case management, rehabilitative, and therapeutic services to residents. Teach, counsel, and monitor residents activities of daily living in a community residence or apartment setting, developing skills, abilities, and experience that will facilitate a greater opportunity for the resident to live in an independent environment.

Provide case management, rehabilitative, and restorative services to residents. To teach, counsel, and monitor residents activities of daily living in a community residence or apartment setting to develop skills, abilities, and experience that will facilitate more significant opportunities for the resident to live in an independent environment.
Minimum education and experience: high school diploma/GED and customer service experience. Healthcare or behavioral health experience is a plus.
Preferred Education and Experience: Bachelors degree in a human services-related field or an associate degree in a human service-related field and three years experience in human services or a high school diploma/GED and five years experience in human service.
Must possess a working knowledge of current issues and concerns of people with mental illness, rehabilitative training methods, and community resources.
OTHER QUALIFICATIONS:
Have a home/cell telephone.
Be able to travel to and among various multi-level facilities with stairs only.
Have a valid drivers license and a safe driving record.
May be required to use personal vehicle for programmatic purposes.
