Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
Tenant Success Service s is Breaking Ground’s newest model of permanent housing service which includes projects that are a part of NYC’s Department of Housing Preservation and Development (HPD) Our Space Initiative as well as private partnerships with developers. Utilizing a Critical Time Intervention (CTI) approach, the program provides "light touch" housing retention services to formerly homeless households during the first year of tenancy, a ssisting with their transition out of the shelter system and into permanent housing.
Reporting to the Assistant Program Director, Tenant Success Services, the Program Coordinator, Tenant Success Services provides supervisory oversight to a new and growing team of Housing Retention Specialists using a Critical Time Intervention (CTI)-informed model; providing time-limited services focused on housing retention, resource development, and community integration with the goal of helping formerly homeless individuals and families prevent eviction and return to shelter.
These services are provided to individuals and families exiting the shelter system and moving into ap artments in new developments located in Brooklyn, Queens , and Manhattan. Th e Program Coordinator collaborates with Tenant Services Coordinato rs who provide ongoing community support to these individuals and families following the first year of tenancy. The Program Coordinator is responsible for ensuring that service provision is of the highest quality and is consistent with CTI-informed principles.
Depending on program need, this position may carry a small caseload of more complex clients. The Program Coordinator liaises with community stakeholders, funders, property management, and real estate development companies, and serves as a member of the Light Touch Services’ management team.
This position has a 35-hour work week .
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide direction and leadership to the program; help troubleshoot, problem solve, and strategize solutions around client engagement and challenging behaviors
Manage caseload assignments and conduct case reviews with staff
Provide training to staff on Critical Time Intervention model
Oversee development and implementation of individual service plans; ensure documentation of client contact and progress is comprehensive and timely
Assess and evaluate client functioning
Supervise and conduct case conferences
Coordinate delivery of care with multiple service providers
Work collaboratively with property management teams as needed to ensure clients succeed in housing and avoid return to homelessness
Provide support and guidance to front line staff in managing difficult situations and successfully de-escalating conflict
Produce timely outcomes reports based on services provided as well as track program participants' entry and exit and monitor for accuracy
Perform other duties as assigned
MINIMUM QUALIFICATIONS:
LMSW preferred; extensive experience working with the target population may be substituted for a master’s degree
Minimum 3 years of experience working with homeless, mentally ill, undocumented, and/or substance using populations
Housing-based case management experience strongly preferred
Previous supervisory experience and strong clinical and assessment skills preferred
Preferred training in CPR and First Aid, Opioid Overdose Prevention, crisis intervention, de-escalation
Familiarity with Critical Time Intervention principles preferred
Proficiency in motivational interviewing techniques preferred
Superb collaborative skills; Ability to work successfully with a wide range of internal and external stakeholders
Creativity, strong problem-solving ability
Ability to delegate and motivate both staff and clients
Ability to communicate (verbally and written) with diverse populations and stakeholders
Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams; proficiency with property management and case management software
Ability to travel throughout the 5 boroughs via public transportation or vehicle
Valid NY or NJ driver’s license with a good driving record, as defined by Breaking Ground is preferred
Bilingual preferred
