Community Health Worker - Delta Health Care Services Reports to Director of Programs
MINIMUM REQUIRMENT:
- Minimum of bachelor's degree in social work, Early Childhood Education or related field.
- Two (2) years' experience in a related field
- two (2) yearsor more as resident in Sunflower County and working knowledge of communoity resources and events.
- Proficient computer skills, including Microsoft, specifically Work and Excel in addition to excellent oral and written communication skills,
- Must have a valid Mississippi Driver's License, proof of insurance and reliable transportation.
RESPONSIBILITIES:
- conduct individual home visits with the expecting mothers, and quarterly group meetings with regularity and consistency, individualizing across diverse families to address individual family needs.
- Manage caseload ( up 50), keeping accurate records and ensuring that all related administrative task are completed and up to date.
- Provide the evidence-base curriculum and emotional support to pregnant wormen and children.
- Maintain and effective support service for pregnant women with the air of enabling the women to develop the skills and resources necessary to help their children develop.
- Design and participate in recruitment activities especially promoting increased awreness of services by education the purblic and linking with other protrams.
- Maintain comfidentiality of all families; communicate orally and in writing to parents, professionals, and other agencies.
- Plan and implemenjt activities for families and children that are culturally sensitive anbd developmentally appropriate both in individuyal and group settings.
