Make a Difference

The Registered Nurse Navigator functions as a member of the ProCare at Home team, conducting duties such as: running reports on patients who are discharging, and recommending appropriate patients for Remote Patient Monitoring. The RN will navigate communication to the home-based services provider and LPN team, collaborate with the home medical equipment team to get monitoring equipment shipped/deployed to the patient's home, build relationships with hospitalists and case management, and other related tasks as needed. The RN Navigator will work on the north side of Indianapolis (main Community North campus), but could have some shifts at the main East campus as needed.

Exceptional Skills and Qualifications

-Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.

-Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).

-Two years or more of nursing experience required.

-Excellent communication skills.

-Excellent team skills.

-Compassionate and caring.

-Accountable and self-directed.

-EPIC experience a plus.
