Make a Difference

The Manager of Population Health Operations for IHCI will oversee operations in quality, risk adjustment, clinical effectiveness, and patient access for value-based care (VBC) and ACO population health management. This includes, but is not limited to:

  • Conduct root-cause analysis to identify performance drivers.

  • Deliver data analysis by evaluating key values and trends for data-driven decision-making.

  • Recommend best practices and workflow changes to improve quality outcomes and efficiencies.

  • Support the IHCI leadership team with data-driven proposals and insights.

  • Collaborate with leadership across population health domains to translate needs into strategic reporting processes.

  • Use technical analytic systems and tools to implement and evaluate initiatives for improving patient outcomes, cost, utilization, and documentation.

Exceptional Skills and Qualifications

The ideal candidate will have a strong understanding of VBC and ACO metrics in various healthcare settings. Candidates can create solutions and lead operational teams to support population health efforts and performance in value-based care programs.

  • Bachelor’s Degree required.

  • Master’s Degree is preferred.

  • In lieu of the above education requirements, a combination of experience and education will be considered.

  • Three (3) years or more experience in population health is required.

  • Three (3) or more years of experience implementing or tracking population health initiatives such as patient-centered medical home, transformation, quality, and/or risk adjustment, ACO development, ambulatory care, and quality and efficiency metrics is required.

  • Five (5) or more years of experience in health care operations is required.

  • Proficiency in data manipulation and reporting functions in Excel.

  • Experience with data extraction using SQL is preferred.
