Make a Difference

The Community Health Coordinator will assess the current nutritional content of foods purchased, served, or sold at Community Health Network hospitals and work with partner agency staff to set benchmarks for increasing nutritional value of foods. The coordinator will make recommendations and allocate resources based on analysis of assessment data and use of professional judgement in the selection or development of the most appropriate nutrition standards and interventions to employ. This role will research and utilize environmental scans and agency food policy and contract reviews appropriate for the food setting and cultural food preferences of consumers, present assessment data to partners, and use professional judgement to make recommendations for improvements. Develop, implement, evaluate, and monitor interventions to improve nutrition policy, systems, and environments appropriate for the food service setting. Communicate and engage business partners in the expansion of charitable food programs across the service area. Instruct partners on the strategies available to improve the nutritional content of foods that are relevant to the needs of the partners and consumers. Document activities performed via monthly reports, electronic calendar, and written summaries of activities.

Exceptional Skills and Qualifications

Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.

· Bachelor’s degree in public health, health education, communication, or a related social, behavioral, or medical sciences field required.

· One year of related experience in health education or public health required.

· Ability to communicate effectively both orally and in writing, including listening.

· Ability to coordinate, lead, prioritize, and/or manage multiple projects competently.

· Efficiently and accurately enter data and organize databases, decision support systems, and other business development information tools.

· Analytical skills to gather, interpret, and organize information for communication plans and publications.

· Demonstrate an understanding of healthcare and health promotion.

· Demonstrates accountability for own actions with an openness to change and learning.

· This position will require traveling, 7330 Shadeland Station, Community Hospital East and Community Heart and Vascular Hospital.

· This position will allow some flexibility to work a hybrid schedule after the initial training period.

· This position is in a partnership and grant funded through the Marion County Public Health Department and Community Health Network through the Racial and Ethnic Approaches to Community Health (REACH) program.
