Indiana also maintains its place as one of the top 5 provider friendly states. This Advanced Practice Provider will join our Bariatric and Medical Weight Loss team at Community Hospital.

Highlights

Community Physician Network is seeking an Advanced Practice Provider, to join the Medical Weight Loss side of our Bariatrics Practice, primarily working at our south location with a day of support at our north location. Both practices for Community Hospital are located in Indianapolis, IN. The Physician Assistant or Nurse Practitioner will work in an office setting providing both initial and follow up consultations with medical weight loss patients which includes prescribing, treating, and educating patients. Additional responsibilities will include patient care coordination with collaborating physicians and Registered Dietitians, care plan documentation, assisting with bariatric program development, support groups, participating in various committees, and tracking of patient work ups

· 1.0 FTE day opportunity

· Monday– Friday, 8:00 am – 4:30pm

· Supporting a dedicated, fulltime Bariatrician with established patient flow

· Electronic Medical Record - EPIC

Qualifications

· Experience in metabolic, nutrition or obesity medicine is strongly preferred but not required

· Ideal candidates will have excellent communication skills and dedication to delivering high quality patient care

· Must have the desire to practice in the operating room, clinic, and inpatient settings

Licensure & Certifications

Educational background consistent with requirements for licensure and certification within the state of Indiana
