Indiana County Community Action Program, Inc.

 

Title of Position: Housing Support Liaison

Immediate Supervisor: Direct Services/Shelter Director

 

 Areas of Responsibility:

  1. Act as a point of contact for coordinating housing information for Armstrong-Indiana Behavioral & Developmental Health Program consumers in Indiana County.
  2. Assess, develop, and coordinate a housing need plan in coordination with the CHIPPS Coordinator at I & A Residential, Inc.
  3. Act as the Armstrong-Indiana Behavioral & Developmental Health Program Liaison in initiating and coordinating referrals for priority consumers to appropriate housing resources and other county housing service providers and social service organizations in Indiana County.
  4. Responsible for correspondence, studies, and reports as required by program procedures.
  5. Responsible for complying fully with all contractual obligations.

     

Specific Duties:

  1. Maintain consumer files with information relating to their housing needs, including the housing need plan.
  2. Develop, implement and monitor polices and procedures for the:
    1. Consumer referral assessment
    2. Master Leasing Rent Program
  3.  Develop a housing need plan with each Armstrong-Indiana Behavioral & Developmental Health Program consumer referral.
  4. Coordinate advocacy support for consumers dealing with housing issues as a result of their mental illness.
  5. Provide services needed by consumers that are not otherwise available, including, but not limited to, life skills education, budget counseling and transportation.
  6. Create a housing assessment tool for case managers, inpatient staff, and other behavioral health professionals.
  7. Participate in local, regional and safe housing committees and work groups to seek out opportunities for involvement in planning and developing housing options for Armstrong-Indiana Behavioral & Developmental Health Program consumers.
  8. Prepare correspondence, studies, and reports associated with administering the housing plan, as required and approved by the supervisor.
  9. Participate and complete SSI/SSDI Outreach, Access, and Recovery (SOAR) Training in order to complete applications for all homeless and/or disabled clients including all other housing/homeless programs.
  10. Attend all training, as required by the supervisor, including, but not limited to, PA HMIS, SOAR Training, etc.
  11. Prepare and distribute informational materials and talk to groups about the BDP Housing Program.
  12. Work as part of a team and maintain a positive attitude
  13.  Other duties as assigned

     

Screening Criteria:

1.   Must possess a B.A. or B.S. Degree.

2.   Self-motivation to complete daily tasks and procedures without close supervision.

3.  Ability to ensure the integrity of data entered.

4.   Willingness to work with team members to achieve desired outputs

5.   Knowledge of the respective county, employment is in Indiana, and familiarity with the    social service network and community resources.

6.   Demonstrated ability to communicate effectively with all members of the community.

7.   Must have basic computer skills, have the ability to utilize computers, including the use    of internet, and the ability to learn software programs.

8.   Good organizational skills and have the ability to meet deadlines in a timely manner.

9.   Must have a valid PA Driver's license, reliable transportation and be willing to travel in   Indiana County and other counties if required.

  1. A Motor Vehicle Report and Act 33 & 34 clearances and FBI clearance with fingerprinting will be obtained upon hire.

  2. Must have vehicle insurance to comply with agency standards of coverage.
