Overview:Adoration Health is seeking a skilled Licensed Practical Nurse (LPN) for a full-time position in Hornbeak, TN. This role involves providing nighttime care for a patient requiring trach and G-tube management. The schedule fluctuates between Sunday to Friday or Monday to Friday, depending on the family's needs. Schedule:

• 50 hours per week

• Fluctuating between Sunday-Friday or Monday-Friday

• Night shifts:8:00 PM - 6:00 AM

Responsibilities:

• Provide high-quality nursing care, including trach and G-tube management, in a home care setting.

• Monitor and document patient conditions, ensuring timely communication with the healthcare team.

• Administer medications and perform routine nursing procedures.

• Collaborate with the patient’s family and healthcare providers to ensure comprehensive care.

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice

• Provide patient and family teaching according to plan outlined by the RN

• Monitor patient's vital signs and make specific measurements and observations

• Monitor and record intake and output. Report significant changes immediately

• Observe, assess and record patient's physical and mental condition and reports any changes

• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly

• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing

• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

• Active CPR certification

• Familiarity with electronic health records

• Strong problem solving and critical thinking skills

• Reliable transportation, valid driver's license and current automobile insurance
