JOB REQUIREMENTS: Adheres to the written Resident Care Plan to: Assist Memory Care residents in maintaining their greatest independence with activities of daily living such as bathing, grooming, toileting and nutritional needs. Also provides assistance with preparing meals, laundry and maintaining a clean, safe physical environment. Assist residents with mobility and transfers Assist residents with medical administration and maintains related reporting requirements. Perform delegated tasks as instructed by the RN, Director of Nursing, Executive Director, Assistant Executive Director, or other management staff. Perform acceptable standard healthcare practices. Assists with housekeeping duties in resident or common areas (vacuuming, dusting, sweeping, cleaning, etc.) as directed. Leads or assists with activities for residents as directed, impromptu and scheduled. Observes and reports resident\'s condition and safety hazards to RN, Director of Nursing, Assistant Executive Director or Executive Director in a timely manner. Documents observations and activity participation in an accurate, complete and timely manner. Maintains excellent work attendance; is punctual for work shift. Possesses organizational skills necessary to carry out shift duties and to complete assigned tasks while promoting residents emotional well-being and ability. Protects residents\' health information according to HIPPA policies and procedures and federal and state law and regulations. Maintains professional appearances, attitude, and demeanor while on duty. Attends all mandatory staff meetings and in-service training opportunities. Performs other miscellaneous tasks as assigned. Administer bedside or personal care, such as ambulation or personal hygiene assistance. Prepare and maintain records of client progress and services performed, reporting changes in client condition to manager or supervisor. Perform healthcare-related tasks, such as monitoring vital signs and medication, under the direction of registered nurses or physiotherapists. Perform housekeeping duties, such as cooking, cleaning, washing clothes or dishes, or running errands. Provide clients with communication assistance, typing their correspondence or obtaining information for them. Turn or reposition bedridden patients. Answer patient call signals, signal lights, bells, or intercom systems to determine patients\' needs. Feed patients or assist patients to eat or drink. Measure and record food and liquid intake or urinary and fecal output, reporting changes to medical or nursing staff. Provide physical support to assist patients to perform daily living activities, such as getting out of bed, bathing, dressing, using the toilet, standing, walking, or exercising. Document or otherwise report observations of patient behavior, complaints, or physical symptoms to nurses. Remind patients to take medications or nutritional supplements. Review patients\' dietary restrictions, food allergies, and preferences to ensure patient receives appropriate diet. Undress, wash, and dress patients who are unable to do so for themselves. Observe or examine patients to detect symptoms that may require medical attention, such as bruises, open wounds, or blood in urine. Lift or assist others to lift patients to move them on or off beds, examination tables, surgical tables, or stretchers. Supply, collect, or empty bedpans. Communicate with patients to ascertain feelings or need for assistance or social and emotional support. Record vital signs, such as temperature, blood pressure, pulse, or respiration rate, as directed by medical or nursing staff. Gather information from caregivers, nurses, or physicians about patient condition, treatment plans, or appropriate activities. Wash, groom, shave, or drape patients to prepare them for surgery, treatment, or examination. Prepare or serve food trays. Change bed linens or make beds. CNA License High School education Twelve months of experience as a personal aide, Caregiver or equivalent life experience Completion of CBRF and CNA certifications (required) Reliability and Flexibility in Scheduling, must demonstrate excellent attendance; must be able to work holidays and weekends and cover as required by resident needs.
