We’re looking for a Community Outreach Navigator to provide quality support and care to others. The Community Outreach Navigator with other Triage team members work to provide community outreach, emergency stabilization, urgent care and assessment services in the community. You are tasked with providing short-term case management supports for clients/families to ensure follow up of services, while helping to facilitate a warm handoff from the Stabilization team to CBHC teams.

Responsibilities:

  • Establish and foster trusting relationships clients and families, support and educate them on accessing the behavioral health system based upon need.

  • Provide overviews of the program and care coordination offered to individuals

  • Administer and track SDOH needs for all new clients encountered.

  • Gather all data, requested through BILH’s evaluation and reporting process.

  • Participate in the completion of the semiannual “progress” reports and annual grant summaries through BILH’s portal/website.

  • Seek out and connect with new key stakeholder groups, primary care clinics, churches, markets, and other essential community support groups that work with the grants target population.

  • Engage with the target population of grant recipients (Hispanic population of Haverhill).

  • Provide clients with harm reduction and overdose prevention education and information.

  • Coordinate with the Jail Diversion Coordinators to promote public awareness and willingness to help connect the target population into behavioral health services. Provide education and activities in community events and school settings.

  • Provide guidance and support to clients in all phases of the recovery process (precontemplation, contemplation, active engagement) in a person centered/strength-based fashion.

  • Promote hope and empowerment through meetings and support with clients through targeted disclosure of your recovery story/experiences.

Qualifications:

  • BA in human services field preferred or high school diploma/ GED with at least five years’ experience working with adults with severe and persistent mental illness. May substitute one year of experience with a college degree in the human services field if a Peer Specialist or Recovery Coach.

  • Must have a strong commitment to mental health recovery, substance abuse disorders, care coordination, and wellness. Must have skills to establish supportive relationships with persons with mental illnesses/substance use and respect for their preferences for treatment.

  • Ability to provide flexible services that are community based and focused on team work.

PAY RATE: $28 per hour (includes 6% bilingual differential rate)
