Job Summary: The HVIP Hartford Collaborative is a three hospital-based violence intervention program serving victims of violence and those exposed to violence and its associated trauma. This position is based at one of three hospitals in Hartford: Connecticut Children’s, Hartford Hospital, and Trinity Health/Saint Francis (travel to each hospital for work is expected). The staff provide inpatient services and resources to help the participant address his/her/their traumatic injury and recovery and provide services to help the participant develop a Safety and Support Plan. Under general supervision, the Violence Protection Professional [VPP] Coordinator provides care coordination services to all patients impacted by violence by supporting and assisting individuals and families with accessing and securing appropriate community services and resources. Works collaboratively with interdisciplinary staff internal and external to the organization to promote health, healing and equity. Demonstrates knowledge of the population-specific differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families and co-workers Job Responsibilities: * Provide comprehensive care coordination services to children, youth, adults, and their families by linking them to appropriate medical, developmental, Behavioral, educational (etc.), and social services and resources. * Utilizes the strength based approach, Strengthening Families™, The 5 Protective Factors Framework, to partnering with families. * In collaboration with individuals and families, and in consultation with primary care providers and specialty providers, develop and implement a comprehensive, shared plan of care that addresses specific needs and goals. * As an integral member of the care coordination team, collaborate with inpatient, outpatient, and behavioral health team members to ensure transitions in care across the continuum that will mitigate the potential for gaps in care, redundancy, duplication, and poor health and wellbeing outcomes. * Participate and advocate for individuals and families during medical appointments, school meetings, case conferences, etc. as appropriate * Participate in all HVIP specific meetings, Care Coordination Collaborative, and various community meetings as appropriate. Meet required HVIP deliverables as indicated. * Provides information, referrals and resources for clients. Assists in the care of complex patients to facilitate the delivery of cost-effective healthcare and in the identification of appropriate utilization of resources. * Participate fully in HVIP activities: staff meetings, work groups, innovative programs and projects, research activities. * Round on patients while they are inpatient. * Demonstrates expertise/skill in one or more areas of care coordination and provide technical assistance to colleagues. Develop and establish relationships with other community and social support providers to aid in effective referrals and timely access to services. * Communicate with providers via fax/email on progress of services. * Document all care coordination activities in data bases/ EMRs as appropriate. * Adherence to current HIPAA guidelines in insure patient/family confidentiality. * Empower families to direct the care of their children within the medical home model (or other care delivery system) by formulating a care plan that promotes self-advocacy, networking skills, and culturally relevant services. * Empower individuals to direct their own care within the medical home model (or other care delivery system) by formulating a care plan that promotes self-advocacy, networking skills, and culturally relevant services. * Provides education, consultation, referrals, training and support to families and community providers through direct advocacy, technical training, and providing educational materials. * Collaborates with colleagues in the Center for Care Coordination by providing information and technical assistance on cases, attending care management meetings, and assisting in the development of center policies and procedures. * Coaches caregivers in developmentally appropriate positive style to provide care. Refers education and outreach program activities, including implementation of appropriate policies and procedures and evaluation of program and services Required * Bachelor Degree in Social Work, Family and Human Development, Public Health or other related fields * Three (3) to five (5)years of experience in program development, community health and/or direct services to individuals impacted by trauma * Must have own transportation and proof of insurance Preferred Certified Violence Prevention Professional (VPP). If not VPP certified, will be required to obtain certification within a pre-determined time. We take great care of careers.
