Come join our Pharmacy team at Saint Francis Hospital, a member of Trinity Health Of New England

Position Purpose

Looking for a motivated individual to support our retail pharmacy operations and serve as a liaison between patients and retail pharmacy for discharge medication prescriptions.

Coordinates with providers and patients to ensure smooth discharges and improved medication access for our patients. Ensures the safe and effective use of pharmaceuticals through their proper storage, handling, evaluation, preparation, compounding, labeling, dispensing, and administration in conjunction with state and federal regulations and departmental policies.

What you will do

  • Prepare, dispense, and supervise the preparation of all medications for patient consumption

  • Assesses need for formulary items ordered accordingly initiates contact with prescriber or benefits provider for substitute

  • Checks for allergies, potential drug-drug interactions/incompatibilities, and adverse drug reactions on all patients before dispensing medication and utilizes appropriate communication channels

  • Verifies all narcotic orders are received, maintains perpetual inventory log, performs weekly C-II inventory and monthly controlled drug inventory

  • Monitors appropriate lab data to ensure patients receive proper dose of medications/nutrition/electrolyte needs and when data is available

Minimum Qualifications

  • Bachelor’s Degree or Pharm D Degree in Pharmacy

  • 6-12 months of previous related experience

  • Licensure required as a Pharmacist by the state of Connecticut
