Job/Position DescriptionQual Intel Disability Prof IQual Intel Disability Prof I is directly accountable for the assessment, development, implementation, and monitoring of a highly individualized Individual Support Plan (ISP) which promotes dignity, respect, choice, and the exercising of personal rights for each person who is on their assigned caseload. The Individual Support Plan (ISP) for each person must be developed by the personand#146;s appropriately constituted Interdisciplinary Team (IDT) through an integrated, collaborative and comprehensive program planning process that reflects the personand#146;s preferences, strengths, needs and personal vision. The QIDP I coordinate the development of programs and training to implement the individual plan for services and supports. Must independently implement Federal/State Standards (ICF/IID), the requirements of the Settlement Agreement with the Department of Justice and the Centerand#146;s Policies and procedures. The QIDP I provide highly complex (senior level) professional and administrative support, and technical assistance work that is designed to address specific needs, desires, and personal goals of those who live in the assigned home(s). The QIDP I work with and interprets needs based on input from a variety of service providers, professionals and clinicians. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Work schedule requires regular flexibility, to ensure effective oversight and monitoring of active treatment programs, services and supports. If needed, may supervise the work of others.Essential Job FunctionsAttends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.Ensures that each individual on the assigned caseload receives assessments and evaluations as identified by the PST for a determination of their current level of functioning, preferences, strengths, needs and recommendations. Facilitates and ensures the timely completion of the Preferences and Strengths Inventory to obtain information on services that are important to the individual to achieve his/her optimal living vision. Coordinates and facilitates the participation of the individual, and as appropriate, the individualand#146;s Legally Authorized Representative (LAR), with the appropriate paraprofessional staff and professional/clinical staff in the development and delivery of needed supports and services.Facilitates, or assists the individual and/or LAR to facilitate, the IDTand#146;s review of the results and recommendations of assessments, evaluations and summaries, and the interdisciplinary discussion to develop an integrated, collaborative, and comprehensive Individual Support Plans (ISP). Completes appropriate documentation to ensure the ISP integrates all supports, services and plans into a single, accessible and comprehensible document. Transmits any needed follow-ups to document decisions of the Interdisciplinary Team.Ensures the implementation, integration and continued appropriateness of the Individual Support Plan (ISP) for each assigned individual. Monitors and provides oversight of the active treatment program of each individual.
