Job Summary:Are you passionate about providing support to individuals experiencing homelessness? Join our team as a Housing Case Manager at Clarity Health!In this role, you will implement Housing First, harm reduction, and low-barrier service models to assist individuals in securing stable housing. Responsibilities include attending Coordinated Entry/Case Conferencing meetings, connecting with households in crisis, maintaining housing grant requirements, and providing support to Low-Income Housing Tax Credit program (LIHTC) property tenants. Key Responsibilities:

  • Develop expertise in community resources and serve as a valuable resource to clients, case managers, and agency administration.

  • Connect referrals with Continuum of Care programs, facilitating transitions to permanent housing options.

  • Attend Coordinated Entry/Case Conferencing meetings to foster collaboration and support for individuals experiencing homelessness.

  • Refer and transport individuals to essential services and support networks.

  • Cultivate relationships with local landlords to promote housing opportunities.

  • Ensure timely data collection and entry for accurate reporting and evaluation.

  • Assist clients in finding housing and accessing related community resources.

  • Support households in transitioning to other permanent housing options.

  • Perform other duties as assigned.

Experience and/or Education Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Sociology, or related field with case manager experience in housing. OR

  • Associate of Applied Science in Behavioral Health Support from an approved institution authorized to offer community support services. OR

  • Qualification to provide Community Support according to DMH guidelines (CS100 Qualifications dmh.mo.gov).

  • Demonstrated strong time management skills, capable of completing paperwork promptly and managing multiple tasks effectively.
