Atlantic Health System Social Worker (MSW) - Part Time in Hackettstown, New Jersey

Job Function 1

  • Conducts comprehensive psychosocial assessments to meet the needs of outpatient oncology population from diagnosis to survivorship

  • Facilitates evidenced based therapeutic interventions to address the complex emotional needs of the oncology family system

  • Ability to triage distress screen and team member referrals for timely response

  • Strong fund of knowledge of county social services and oncology specific resources

  • Develop and facilitate support groups and psychoeducational programming

  • Communicates with interdisciplinary AHS team members across the system to ensure continuity of patient care

  • Assures timely documentation in EPIC

Job Function 2

  • Conducts daily rounding and communicates with nurses and other team members.

  • Collaborates with physicians, nurses, navigators, social workers, care managers, etc. to address patient needs and care plans. Follow up on referrals from other members of the team and reports back to them as appropriate

  • Participates in relevant committees and programs (e.g., Cancer Committee, Tumor Board etc.) as requested.

  • Supports patient satisfaction initiatives (press ganey and HCAPS)

  • Collaborates with patient liaison on patient support and other patient initiatives.

  1. Job Function: Coordinates Educational and Support Programs
  • Collaborates with other Oncology Services and staff to design, implement and evaluate supportive care services (support groups, educational seminars, etc.) to assure services address the continuum of cancer care

  • Creates and implements new programs/services to address patient and family needs

  • Maintains knowledge of existing resources and services and makes referrals to community resources as appropriate.

  • Generates a monthly narrative/statistical report on services provided

  • Facilitates groups with other disciplines to provide balanced clinical care as needed or appropriate to role

  • Provides education to the community about psychosocial related topics through lectures and educational programs as requested or appropriate

  1. Job Function: Ensures that a high level of professional development is maintained
  • Identifies professional learning needs

  • Participates in in-service learning programs regularly

  • Attends outside workshop and conference to improve knowledge and skill level

  • Completes requirements for maintenance of current of professional license

Required:

  1. MSW from an accredited school of social work.

  2. Active LSW or LCSW required.

  3. Experience in medicine and chronic disease population strongly preferred.

    Hackettstown Medical Center has been providing care to the community since 1973, primarily serving Warren, Sussex and Morris Counties in New Jersey.
