  What you will do in this role:

  In this expanded healthcare role, you will act as a liaison between patients & their families, primary care providers, specialists, and other members of the health care team. You will ensure the delivery of quality, efficient, and cost-effective health care services for those with complex conditions and complex social needs.

  What’s Required:

• Licensed as a Registered Nurse in the State of Michigan.

• Bachelor of Science degree in nursing preferred.

• Five years’ experience as an RN preferred.

• High-level understanding of complex chronic disease management.

• Self-directed professional with the ability to make independent judgments directing patient care.
