Social Worker - Admission Case Manager

Duties and responsibilities include but are not limited to: Provides case management and care coordination of health care services to Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges? ?Links Veterans with services, resources, and opportunities, to maximize their independence, health, and well- being ?Acts as an advocate with appropriate community service providers/agencies ?Provides professional, independent social work services to Veterans who are experiencing issues related to alcohol or substance use disorders and who are experiencing a wide range of complex medical, psychiatric, emotional, behavioral, and psychosocial problems?

Requirements: United States Citizenship Masters Degree in Social Work with fully accredited by the CSWE Current, full and unrestricted Social Work license from any US state or District of Columbia
