SCL Health Psychiatric Liaison in Grand Junction, Colorado

Job Description:The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties. This position provides care appropriate to the established plan of care in the assigned patient population.

As a Psychiatric Liaison / Clinical Social Worker you need to know how to: Provide acute emergency services and clinical evaluation of psychiatric and other ED patients in order to expedite timely throughput or discharge from the ED. Provide pro-active initial and ongoing clinical assessments of patients psychiatric, medical, financial, and functional needs to assist in development of a care plan to ensure safe and timely discharge. Coordinate with both internal and external care team members to ensure a safe transition to the next level of care through planning and implementation of both skilled services and community resources. Serve as the clinical resource for the Emergency Department staff and provide guidance to the physicians regarding the laws and requirements of mental health holds. Serve as the liaison between the organization and outside resources with the ultimate goal of quality psychiatric patient care within the community. Educate patients/family/caregivers and organizational staff on medical necessity, appropriate level of care and alternatives for care.

Minimum Qualifications Completion of a Masters Degree in Social Work (MSW) from an accredited program Minimum of three (3) years of experience in a comparable setting Licensed Clinical Social Worker (LCSW) / Licensed Professional Counselor (LPC) within two (2) years of hire

Preferred: Current Licensed Clinical Social Worker (LCSW) / Licensed Professional Counselor (LPC)

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.) May be expected to stand in a stationary position for an extended period of time.
