Description

REPORTS TO: Medical Director / Practice Manager

PAY RANGE: $99,200 - $123,000 per year

WORK SCHEDULE: Full-time (40 hours). Weekdays only. No evenings, weekends, or on-call.

POSITION SUMMARY: Provides Family Practice services to individuals of all ages.

PRIMARY RESPONSIBILITIES:

  • Conducts initial examinations with recommendations for treatment and follow-up care to individuals.

  • Conducts follow-up examinations with recommendations for any changes in the care plan.

  • Prescribes medications as condition indicates within the appropriate scope of licensure.

  • Make appropriate referrals when necessary.

  • Discusses the medical area of the treatment plan with the individual, parents, relatives/advocates, or guardians.

  • Serves as a member of the interdisciplinary clinic team to ensure that the medical needs of patients are met.

  • Maintains confidential medical records.

  • Ensures referral and/or consultation with a collaborating physician, as appropriate.

  • Assures compliance with all regulations of health and clinical services.

  • Ensures standards of medical care.

  • Assists nursing staff in providing emergency first aid, crisis intervention, and infection control procedures.

  • Ensures review of medical care provided and recommended by collaboration physician.

  • Performs any other duties agreed upon with the Medical Director.

Requirements

  • Current, unrestricted New York State licensure as Physician Assistant or Nurse Practitioner

  • Curent Board certification

  • At least one year of experience as PA or NP preferred
