Alternatives, Inc. Case Manager - Franklin House in Franklin Township, New Jersey

For over 40 years, Alternatives, Inc. has been dedicated to providing the best in support services to adults with Developmental/Intellectual Disabilities and Mental Illnesses. Services include Residential, Behavior Support, Support Coordination and Recreation.

The Franklin House Transitional Housing Program homeless women and their children in a home like atmosphere. The families are each provided a private and comfortable living space in which they reside for approximately 12 months. During this period of time, the women are involved in educational programs, build work-readiness skills, secure employment, gain parenting, financial and other daily living skills. Staff are on-site 24/7 to provide services and support as needed. - add

    Qualifications:

  • High school degree required, Associates or Bachelors degree preferred - add

  • Ability to pass criminal/registry background checks/drug screen

  • A valid driver's license, no more than 5 points or DUI in last 3 years

  • Ability to pass and maintain required Agency and program- specific trainings- add

  • Experience working with the homeless population preferred- add

    Essential Duties and Responsibilities of a Case Manager:

  • Assist clients in developing goals oriented towards gaining self-sufficiency, parenting skills, finding permanent housing, etc.

  • Provide on-site services and support, including provision of direct service to clients in areas such as money management, daily living skills, psycho-education, work readiness skills, etc., in order to be able to transition into permanent housing

  • Provide transportation as needed to assist with child care, grocery shopping and other needs in the community.

  • Support families personal outcomes to enhance quality of life

  • Utilize an electronic health record (EHR) system to write clear and concise progress notes

  • Ability to pass all Agency and program specific trainings

    Training:

  • All employees are required to complete 10-15 trainings upon hire and on an ongoing basis.

  • Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting.

    4 shifts available :

    PT: Monday, Wednesday, Friday, 9am - 5pm

    PT: Tuesday, Thursday, 8am - 5pm & Sunday, 7am - 3pm

    PT: O/N Monday & Wednesday, 10pm - 8am

    PT: O/N Tuesday, Thursday, 10pm - 8am & Saturday, 10pm - 7am

    $16.00/hr

    BA & Experience Preferred

    Computer skills a must !

    Must possess valid NJ/NY/PA driver's license

    In order to be eligible for employment, applicant must not have more than 5 points on their driving record and no DUIs within the last 3 years.
