South Middlesex Opportunity Council Community Support Service Case Manager in Framingham, Massachusetts

SUMMARY

Provide care management services in a program serving chronically homeless single adults and high utilizers of emergency services. Provide housing search and access to rental assistance programs. Case management includes initial and ongoing assessment with regular follow up and referrals/linkages to community based services. Perform home visits & provide group facilitation.

PRIMARY RESPONSIBILITIES

  • Ensure adherence to policies and protocols for programs serving homeless individuals, including guidelines for meals, curfews, meeting attendance, client transportation to appointments, etc.

  • Ensure clients have primary medical care providers, behavioral healthcare, substance use recovery resources, access to workforce development programs, application for mainstream benefits (SSI/SSDI, EAEDC, SNAP), transportation, food resources

  • Promote harm reduction strategies and provide education on smoking cessation, exercise, nutrition, and ADL's

  • Ensure all participants meet the requirements for entry into the program and that all required documentation is included in the client file.

  • Gather documents necessary for obtaining a housing voucher, application and move-in process.

  • Conduct initial service needs assessment with all clients entering the program. Develop Individual Service Plan, Safety Plan and budget with each participant on entry into the program.

  • Work closely with shelter staff to identify new program participants based on eligibility.

  • Maintain daily contact with clients actively involved in the program.

  • Establish and maintain regular contact schedule with all participants.

  • Maintain appropriate files and minimum weekly case notes for each participant.

  • Ensure accurate statistical data is maintained and reported monthly.

  • Outreach to treatment programs and other community-based resources to establish contacts for client referral; keep detailed log of all referrals made.

  • Provide crisis intervention

  • Engage all clients by understanding and addressing their needs whether within or outside the scope of work.

  • Attend & participate in engagement team meetings as requested and communicate effectively with clients and staff in other areas.

  • Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements.

  • Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures.

  • Other duties as assigned.

    KNOWLEDGE AND SKILL REQUIREMENTS

  • Bachelor's Degree in related field or relevant life experience working with the target population.

  • Substance abuse experience required.

  • Excellent interpersonal and organizational skills.

  • Previous experience with group facilitation preferred.

    ORGANIZATIONAL RELATIONSHIP

  • Directly reports to Community Support Service’s Program Manager.

  • Direct reports of this position are none.

    PHYSICAL REQUIREMENTS

  • Ability to drive a vehicle.

  • Ability to ascend/descend stair.

    WORKING CONDITIONS

Travel within the state will be required. As part of the responsibilities of this position, the Case Manager will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services. A successful background check is required.
