Parkview North Hospital Palliative Care Coordinator in Fort Wayne, Indiana

Summary

Under the direction of department leadership, administers the day to day tasks of the Palliative Care Team relating to overall coordination of patient flow and services delivered to patients and families while working collaboratively with all members of the multi-disciplinary team. Manages daily huddle consisting of Providers, Chaplain and MSW staff with review of existing patients. Manages physician referrals, triage and assignment of new in-patient consults. Performs chart review with creation and preparation of EPIC consults and progress notes for providers. Educates patients on Palliative Care services and coordination of care for candidates of Palliative Care O/P Clinic. Identifies appropriateness of patients being referred for O/P clinic care. Insures continuity of care between settings, physicians and care teams. Responsible for providing quality patient information to providers regarding referrals, consult reason, patient location, medical history and current health issues. Responsible for the correct creation of EPIC charting for in-patient consult and progress notes with the required medical documentation.

Education

Must have completed a medical assistant program. Associate's or Bachelor's Degree in a healthcare related field preferred.

Licensure/Certification

Medical Assistant certification required.

Experience

3 - 5 years of related Healthcare experience. Management or program management experience preferred. Experience in medical setting with working knowledge of case management systems.

Other Qualifications

Must have good verbal, written, communication skills. Demonstrates organizational skills and ability to function autonomously. Must be able to demonstrate proficiency in Word and Excel. Must be organized, able to communicate effectively with others, able to work well on a multidisciplinary team and have excellent time management skills. In addition, should be aware of privacy laws and understand the need to maintain patient confidentiality.
