Queens Community House Family Support Coordinator - Forest Hills in Forest Hills, New York

The Family Support Coordinator (FSC) is a position created to help us respond to the requests coming from community members who might be dealing with issues of insecurity, job loss, housing instability, social isolation, domestic violence, health/mental health concerns, and emergency needs. The FSC will assess incoming requests and make referrals, as appropriate to QCH staff or external resources. The FSC will also offer backup support to line staff, as needed, and directly handle cases that do not fit squarely into one of QCH’s existing program areas.

Primary Responsibilities:

  • Provide a comprehensive and integrated case management service delivery model that promotes individual, household, and community well-being and helps families who are facing a complex array of challenges to develop and implement a comprehensive family plan.

  • Provide short-term case management to individuals and households. If individuals and households need intensive support, provide care coordination over an extended period to support households’ sustained engagement in external services and progression toward family and individual thriving.

  • Introduce households to the range of Queens Community House’s existing programs/services, and offerings including: youth employment, teen center, food pantry, college/career advisement, adult literacy, food stamp enrollment- SNAP – financial assistance assessments/referrals, after school, senior centers, family daycare services and health access;

  • Support QCH program directors in their efforts to identify, track, and respond to needs presented by our participants and their families;

  • Oversee work at morning pantry once a week to assess for households in need of additional support;

  • Participate in and actively contribute to cross-department meetings to support agency wide assessment, planning, and interventions in support of our families and neighbors;

  • Develop relationships with community partners who provide mental health, special needs, and other support services for families.

  • Track and document all inputs, outputs, milestones, and outcomes related to the program in accordance with QCH’s and supervisor’s requirements.

  • Keep track through Salesforce application of all requests, referrals, and responses and produce regular reports;

  • Participate in weekly case conferencing calls with other staff doing similar work.

Qualifications:

We are looking for someone who:

  • Required MUST be fluent in Spanish;

  • is committed to the multi-service, settlement house model;

  • has experience in individual case management, group work, activity management, and mediation.

  • Is familiar with current best practice models of family engagement work;

  • Has Bachelor’s degree in Social Work, Psychology or a related field and three (3) years of professional experience; or a high school diploma and five (5) years of professional experience in community-driven programs working directly with children, youth, and families; MSW preferred;

  • can work independently while maintaining appropriate communication with supervisor;

  • Work in a fast-paced environment on multiple projects occurring simultaneously.

  • Must have a high level of interpersonal skills to handle sensitive and confidential situations.

  • Is able to apply principles of logical thinking to a wide range of intellectual and practical problems;

  • is willing to travel within the network catchment area;

  • has strong knowledge and familiarity with various computer programs on Microsoft Office Suite including Outlook, Word, Excel, and PowerPoint;

  • Prior experience with data-tracking software such as Salesforce;

  • Proven track record of excellent follow-through on assignments;

Status: Full-Time, Non-Exempt

Salary: $27.47 hourly ($50,000 per annum)

Schedule: 35 hours per week
