PROGRAM DIRECTOR COMMUNITY HEALTH ACCESS PROGRAM (CHAP) Greater Flint Health Coalition -- Flint, MI

Overview: The Greater Flint Health Coalition (GFHC) is seeking an experienced Program Director for the Mid-Michigan Community Health Access Program (CHAP). Launched in 2015, Mid-Michigan CHAP is a community-based population health program serving a diverse network of healthcare providers, physician organizations, health plans, and hospitals. The program aims to connect residents to medical homes, address social determinants of health, improve care quality, and reduce costs. The primary population served includes children and adults covered by Medicaid, as well as uninsured and underinsured residents.

The Program Director will lead a multidisciplinary team of licensed social workers, registered nurses, and certified community health workers. Responsibilities include connecting clients to medical homes, navigating insurance enrollment, and addressing barriers to care such as transportation, food insecurity, and financial assistance. The Director will engage with physician practices, health insurers, hospitals, government agencies, and community organizations to expand the program's reach.

This role is based in Flint, MI, within a fast-paced, outcome-driven environment. GFHC's mission is to improve the health status of area residents and enhance the quality and cost-effectiveness of healthcare systems in the community.

Primary Responsibilities:

  • Provide leadership and administrative structure to achieve program outcomes aligned with GFHC's strategic objectives, grant and health plan contract deliverables, and partnership agreements.
  • Lead CHAP managers and collaborate with GFHC leadership and stakeholders to design, implement, and achieve program outcomes.
  • Evaluate and report performance metrics related to client engagement, partner referrals, and program outcomes.
  • Oversee CHAP operations, including referral/intake systems, caseload management, intervention strategies, and staff assignments.
  • Develop and maintain CHAP operations, training, and support manuals, promoting a culture of excellence.
  • Supervise CHAP managers and program staff, ensuring consistent, respectful, and effective client engagement.
  • Partner with GFHC leadership to plan and execute committee meetings and work closely with medical practices to ensure continuous referral volume.
  • Develop practice engagement strategies, address arising issues, and coordinate problem-solving efforts.
  • Manage contracts, business associate agreements (BAAs), and grant agreements with Medicaid Health Plans, healthcare partners, and grant entities.
  • Develop new partnerships and contracts aligned with CHAP's strategic objectives.
  • Collaborate with GFHC administration on budget and staffing to ensure program sustainability.
  • Engage in advocacy efforts for improved healthcare access and quality, and represent Genesee County in the Michigan CHAP (MI-CHAP) network.

Qualifications & Experience Required:

Master's degree in healthcare, public health, education, or related field (LMSW preferred).

5+ years of experience leading and managing care management or community health improvement programs.

5+ years of experience supervising programmatic or clinically licensed staff.
